Infection of a cerebrospinal fluid system is a serious medical complication. We performed a retrospective monocentric analysis on temporary and permanent cerebrospinal fluid devices in children with and without cancer, covering a period of over 14 years. Between 2004 and 2017, 275 children with a cerebrospinal fluid system were seen at our institution. Thirty-eight children suffered from 51 microbiologically proven infectious episodes of the cerebrospinal fluid system (12 children with cancer and 26 children without cancer). Independently of the cerebrospinal fluid system used, the incidence of infection did not significantly differ between children with and without cancer and was the highest in children younger than one year. Infection occurred earlier in external ventricular drain (EVD) than ventriculoperitoneal (VP) shunt, and in EVD significantly earlier in children with cancer compared with patients without cancer. The pathogens isolated were mainly Gram-positive bacteria, in particular Staphylococcus spp., which should be taken into account for empirical antimicrobial therapy.

Background: External ventricular drainage (EVD) is common after brain surgery. However, the incidence of EVD-related infection (ERI) is still relatively high and can increase morbidity and mortality.Objective: The objective of this study was to analyze ERI factors in pediatric population post-brain tumor surgery.Methods: From January 2016 to December 2017, 147 patients <18 years old underwent tumor removal at Beijing Tiantan Hospital and had postoperative EVD. We recorded basic demographic data as well as several risk factors. We then analyzed whether these factors were related to ERI.Results: Patients with a preoperative ventriculo-peritoneal (V-P) shunt, those with longer operation time, those who received blood transfusion, those with more frequent cerebrospinal fluid (CSF) sampling, and those with longer indwelling time of EVD had higher risks of infection (p < 0.05). Logistic regression analysis confirmed that a preoperative V-P shunt, operative duration, intraoperative blood transfusion, frequency of CSF sampling, and EVD duration were correlated with postoperative ERI (p < 0.05).Conclusion: EVD should be removed as soon as possible and any unnecessary procedures should be avoided to reduce the infection rate. However, prophylactic treatment should be given in case patients do not meet the indication for EVD removal.

Tikslas. Nustatyti pooperacinio bakterinio meningito dažnį, rizikos veiksnius, sukėlėjus bei ligos baigtis pacientams, gydytiems Lietuvos sveikatos mokslų universiteto ligoninės (LSMUL) Neurochirurgijos intensyvios terapijos skyriuje (NITS) po išorinio skilvelio drenažo (ISD) operacijos. Tyrimo metodika. Atliktas retrospektyvusis tyrimas, į kurį įtraukta 212 pacientų, kurie 2012-2016 metais po ISD suformavimo operacijos buvo gydyti NITS. Analizuoti tiriamųjų demografiniai rodikliai, indikacijos ISD operacijai, rizikos veiksniai pooperaciniam bakteriniam meningitui išsivystyti, gydymo trukmė NITS ir ligoninėje, infekcijos sukėlėjai bei ligos baigtys. Rezultatai. Pooperacinis bakterinis meningitas po ISD suformavimo operacijos buvo nustatytas 50 (23%) tiriamųjų. Dažniausi meningito sukėlėjai – stafilokokų rūšies patogenai. Rizikos veiksniai pooperaciniam meningitui išsivystyti: ISD kateterio laikymo trukmė >10 dienų (p=0,034), pakartotinis ISD kateterio įkišimas (p&lt;0,001), nustatyta imunosupresinė būklė (p=0,006), sąmonės būklė pagal Glasgow komos skalę (GKS) &lt; 9 balų (p=0,029). Pooperacinis bakterinis meningitas ilgina tiek gydymo NITS (16,2 ± 14,06 dienos ir 11,2 ± 8,93 dienos, p=0,029), tiek ligoninėje trukmę (44,12 ± 27,01 dienos ir 22,36 ± 20,58 dienos, p=0,004). Išvados. Pooperacinio bakterinio meningito dažnis po ISD operacijos 23 proc. Pagrindiniai pooperacinio bakterinio meningito rizikos veiksniai: ISD kateterio laikymo trukmė daugiau nei 10 dienų ir pakartotinis drenažas. Pooperacinis bakterinis meningitas ilgina gydymo NITS ir ligoninėje trukmę.
