Infection with cytomegalovirus in homosexual men.

Abstract: SUMMARY The prevalence and incidence of cytomegalovirus (CMV) infections were studied in a group of homosexual men. Of the 710 participants, 501 (70-6%7o) had complement fixing antibodies to CMV on entry to the study. During the follow up (maximum 23 months) 69 CMV infections were found: 50 primary infections among the 209 seronegative men (attack rate 27 -3 0o), and 19 recurrent infections among the 501 seropositive men (attack rate 6-2%7o).The prevalence of antibody to CMV was correlated with four character… Show more

“…1 In the MSM population, the number of partners, history of other sexually transmitted infections, anal contact, and lifetime duration of sexual activity all correlate with the risk of CMV acquisition over time. 2 Although CMV IgM may be nonspecifically elevated during reactivation, this patient's significantly elevated CMV IgM level in the context of a new sexual contact, an appropriate latency period, violent gastrointestinal symptoms, prominent involvement of the rectum, rash, markedly elevated CMV viral load, atypical lymphocytosis, and abnormal liver function tests strongly supported a diagnosis of primary CMV infection acquired via unprotected receptive anal intercourse. CMV IgG was not obtained, given the duration of symptoms at presentation.…”

“…There is a vast diVerence between the frequency of seropositivity in the gay male compared with hetereosexual males of the same socio-economic stratum (94 vs 54%, [27]). CMV seropositivity or latent infection was correlated with duration of sexual activity, number of sexual partners, history of syphilis and anal intercourse [28,29]. Interestingly, there is also evidence of continuous lytic infection activity in the immunocompetent healthy sexually active gay male.…”

“…Mintz et al (1983) found that of seven different practices of the men, only passive analgenital intercourse correlated significantly with the acquisition of CMV infection. Coutinho et al (1984) and Collier et al (1987) found that seropositivity correlated with duration of sexual activity, nurober of sex partners, history of syphilis, and anal intercourse. The acquisition of primary CMV infection was only correlated with a history of syphilis and anal intercourse.…”
