Background: Concurrent epidemics of both dengue fever and leptospirosis are known to occur in the tropics; however, it is unclear whether coinfection is associated with different clinical manifestations and outcomes. Materials and Methods: Medical records for patients with serological evidence of leptospirosis, dengue, and coinfection from January 2007 to December 2008 were abstracted using a standardized abstraction form. The patients' demographic data, underlying diseases, clinical manifestations, laboratory data, and disease outcomes were retrospectively analyzed. Results: The frequency of leptospirosis was 15.8% (287/1814), while that of dengue was 14.8% (239/1610) and coinfection was seen in 1.3% (17/1309). Highest mortality of 29.6% (5/17) was seen in coinfected cases, followed by leptospirosis (14.6%) and dengue (3.7%). Conclusion: In the tropics, leptospirosis and dengue can coexist and increased awareness of coinfection, and an enhanced ability to diagnose it early in illness, will help guide the appropriate use antibiotics and supportive measures to reduce mortality.

Abstract. Co-infection with pathogens that cause acute febrile illness creates a diagnostic challenge as a result of overlapping clinical manifestations. Here, we describe four fatal cases of Leptospira species/dengue virus co-infection in Puerto Rico. Although all patients sought care early, antibiotic administration was delayed for most. Steroids were administered to all patients, in most cases before antibiotics. These cases show the need for clinicians evaluating patients in or recently returned from the tropics with acute febrile illness to consider both dengue and leptospirosis. Furthermore, they illustrate the need for nucleic acid-or antigen-based rapid diagnostic tests to enable timely patient diagnosis and management. In particular, antibiotic therapy should be initiated early for patients with suspected leptospirosis, and steroids should not be administered to patients with suspected dengue.

Co-infection in immunocompetent patients is rare. Though co-infection with dengue and leptospira cases is increasingly reported, a co-infection of this combination along with hepatitis E is rarely thought of. Until date only two case of triple co-infection have been reported world-wide. Here, we are reporting a patient with co-infection of dengue, leptospirosis and hepatitis E admitted to our intensive care unit. Early septic shock and increasing procalcitonin in dengue patient raised suspicion of co-infection. Our aim is to educate intensivists about this rare co-infection and hence that timely initiation of appropriate diagnostic, therapeutic and supportive measures can alter outcome favorably.
