Objectives: The aim of this study was to evaluate the efficacy on crying episodes owing to infantile colic of a new infant formula containing partially hydrolysed whey proteins, prebiotic oligosaccharides (OS), with a high b-palmitic acid content. Design: Prospective randomized controlled study. Setting: Italy. Subjects: Two hundred and sixty-seven formula-fed infants, aged less than 4 months, with infantile colic, were randomized to receive either the new infant formula (study treatment (ST)) or a standard formula and simethicone (6 mg/kg twice a day) (control treatment (CT)). A questionnaire was given to parents to evaluate for 14 days the daily number of colic episodes and crying time. Results: Out of the 199 infants who completed the study, 96 were treated with the new formula and 103 were not treated. Infants receiving the new formula had a significant decrease in colic episodes after 1 week (2.4771.94 at day 7 vs 5.9971.84 at the study entry) compared to infants receiving the CT (3.7271.98 at day 7 vs 5.4171.88 at the study entry) (Po0.0001). Also at day 14, the crying episodes were significantly different between the two groups of infants (1.7671.60 in ST vs 3.3272.06 in CT) (Po0.0001). Conclusions: The use of a partially hydrolysed formula supplemented with fructo-and galacto-OS induces a reduction of crying episodes in infants with colic after 7 and 14 days when compared with a standard formula and simethicone.

WHAT'S KNOWN ON THIS SUBJECT: Infantile colic affects almost 10% of all infants and is characterized by crying and fussing in an otherwise healthy and well-fed infant. Prenatal exposure to tobacco smoking is a risk factor, but it is unclear whether nicotine causes the association.WHAT THIS STUDY ADDS: Infants exposed to nicotine replacement therapy during pregnancy had elevated infantile colic risk of the same magnitude as infants exposed to tobacco smoking. Intrauterine exposure to nicotine may play a causal role in the pathogenesis of infantile colic. abstract OBJECTIVE: To investigate the associations between use of nicotine replacement therapy (NRT) and smoking during pregnancy and infantile colic in the offspring. METHODS:We used data from maternal interviews (from pregnancy and at 6 months post partum) from the Danish National Birth Cohort (1996)(1997)(1998)(1999)(2000)(2001)(2002). We included 63 128 live-born singletons with complete information on nicotine exposure during pregnancy and infantile colic symptoms as recorded at 6 months of age. RESULTS:A total of 46 660 infants (73.9%) were unexposed to nicotine during pregnancy; 207 (0.3%) were exposed to NRT, 15 016 (23.8%) were exposed to smoking, and 1245 (2.0%) to both. A total of 4974 (7.9%) infants fulfilled Wessel' s modified criteria for infantile colic. Prenatal nicotine exposure was associated with elevated risk for infantile colic in the offspring. Compared with the unexposed, NRT users had an adjusted odds ratio (OR) (95% confidence interval) of 1.6 (1.0-2.5; P = .03), smokers had OR = 1.3 (1.2-1.4), and women who both smoked and used NRT had OR = 1.6 (1.3-1.9). Partners' smoking was not associated with infantile colic after adjustment for maternal smoking. CONCLUSIONS:We corroborated the association between smoking and infantile colic after adjustment for several possible confounders in a large cohort study. Moreover, we found that infants exposed to NRT prenatally had an increased risk for infantile colic of the same magnitude as those exposed to tobacco smoke. У детей первого года жизни нередко возникают функциональные нарушения пищеварения (срыгивания, запоры, кишечные колики, метеоризм), которые в ряде случаев становятся причиной обращения родителей к врачу-педиатру [1,2]. Это нередко приводит к замене одних детских молочных смесей на другие, поскольку среди родителей (и отчасти среди врачей) бытует мне-ние, что подобная симптоматика означает «неперено-симость детской молочной смеси». Однако показатели роста и развития детей в большинстве случаев остаются в пределах возрастной нормы. СРЫГИВАНИЯ (РЕГУРГИТАЦИЯ)Ежедневно возникающие срыгивания достига-ют частоты 86,9% у детей двухмесячного возраста, а к 1 году частота их снижается до 7,6% [3]. Наличие срыгиваний зависит от объема пищи: чем больше объем кормления, тем более продолжительна эва-куация пищи из желудка и выше внутрижелудочное давление, вследствие чего чаще возникает спонтан-ная реакция нижнего пищеводного сфинктера, кото-рая способствует появлению гастроэзофагального рефлюкса (ГЭР).
