Infantile Abdominoscrotal Hydrocele: A Not So Benign Condition

Abstract: Tense infantile abdominoscrotal hydroceles are associated with a high rate of testicular dysmorphism, which is often reversed by early intervention. Simple transscrotal plication of the tunica vaginalis represents an effective procedure with decreased postoperative complications.

“…The PV develops within the gubernacular mesenchyma that invades and migrates into the scrotum, and provides a potential space into which the testis descends. The regression of gubernaculum testis and the obliteration of the PV normally occur soon after the testicular descent has been completed (Cozzi et al, 2008). Therefore, both events may occur in response to the same stimuli.…”

“…If, during that period, eventual complications are noticed, such children should go to surgical treatment. Most frequent approach used in surgical treatment of abdominoscortal hydrocele is ingvinal approach (3) which we used in case of our patient, though there are reported cases that were treated in scrotal way with less invasive surgical treatment (1,14,15) and laparoscopic methods (16). Selection of surgical technique is individual in terms of the approach to surgical treatment of patient with abdominoskrotal hydrocele.…”

“…Most of testicles with dismorphosis achieved in post-operative period a complete recovery (1). In the light of these facts, surgical treatment is preferable type of treatment for patients with abdominoscrotal hydrocele, because the approach is the least invasive, post-operative complications are rare, and eventual consequences associated with abdominoscrotal hydrocele are avoided (3,15).…”

“…This rare type of hydrocele is bilateral in 30% of cases and can usually be diagnosed on physical exam or even prenatal ultrasound, although scrotal ultrasound can confirm the diagnosis. Surgical treatment is traditionally through an inguinal approach, but there is a movement towards scrotal approach due to concern for potential cord damage with the former approach (Belman 2001;Cozzi et al 2008). Testicular and epididymal anomalies are common with these hydroceles, although many resolve after hydrocelectomy (Bayne et al 2008;Cozzi et al 2008).…”
