Filicide, a highly traumatic act, requires very specific meanings. In the international literature a large proportion of cases are correlated with maternal mental pathology. The case we report describes a tragic filicide by a mother with no psychiatric history. This acting out reveals schizophrenia. Thus, the main objective of our work is to emphasize the need for the diagnosis and early management of psychiatric illness, in particular in pregnant women in order to provide violence oneself or to others and to raise the problem of social prevention for these patients and their children and families.

L’infanticide est un crime connu depuis les temps anciens. Il suscite émoi, répulsion et incompréhension car venant souvent de la personne censée la plus soucieuse du bien-être de l’enfant. Ses causes peuvent être d’origine psychopathologiques mais les contraintes socio-économiques et culturelles jouent un rôle non négligeable d’où la grande variation de ses présentations cliniques. Nous avons choisi de présenter le cas d’une femme de 26 ans, sans suivi psychiatrique antérieur à l’acte, qui a tué son enfant âgé de trois mois en le jetant pardessus la fenêtre du troisième étage de son domicile. L’examen psychiatrique a relevé des antécédents de signes anxieux non spécifiques remontant à plusieurs années avec une décompensation psychotique trois jours avant l’acte. L’approche légale de l’infanticide tend à atténuer la responsabilité de la mère au vu des modifications psycho-physiologiques et hormonales engendrées par l’accouchement. La détection précoce des troubles mentaux durant la grossesse et du post-partum et leur traitement pourrait diminuer l’incidence de ce crime.

Suicide and infanticide are the most severe complications of postpartum disorders such as depression and psychosis. Infanticide is more commonly correlated with a younger age of the mother, economic stress, unemployment and a history of psychiatric disorders. Infanticide may take several forms such as the altruistic infanticide, associated with the subsequent mother's suicide, may be in relation with acute psychotic symptoms of the mother, or it can occur in cases of fatal maltreatment of an unwanted child. We present the case of a 40 years old women diagnosed with post partum depression (PPD) who committed infanticide after the birth of her seventh child. The patient had a history of psychiatric disorder in relation with previous pregnancies for some of which she sought medical attention but did not follow a constant treatment. The management of the case was a challenging one with an initial cross-sectional diagnosis established upon admission (e.g post-natal depression) and a final one, established comprehensively by the therapeutic team, which included the current episode within the general psychopathological framework alongside with the personality traits and social context.
