Background: Medical and nursing education lack adequate practical nutrition training to fit the clinical reality that health workers face in their practices. Such a deficit creates health workers with poor nutrition knowledge and child undernutrition management practices. In-service nutrition training can help to fill this gap. However, no systematic review has examined its collective effectiveness. We thus conducted this study to examine the effectiveness of in-service nutrition training on health workers’ nutrition knowledge, counseling skills, and child undernutrition management practices.Methods: We conducted a literature search on nutrition interventions from PubMed/MEDLINE, CINAHL, EMBASE, ISI Web of Knowledge, and World Health Organization regional databases. The outcome variables were nutrition knowledge, nutrition-counseling skills, and undernutrition management practices of health workers. Due to heterogeneity, we conducted only descriptive analyses.Results: Out of 3910 retrieved articles, 25 were selected as eligible for the final analysis. A total of 18 studies evaluated health workers’ nutrition knowledge and showed improvement after training. A total of 12 studies with nutrition counseling as the outcome variable also showed improvement among the trained health workers. Sixteen studies evaluated health workers’ child undernutrition management practices. In all such studies, child undernutrition management practices and competence of health workers improved after the nutrition training intervention.Conclusion: In-service nutrition training improves quality of health workers by rendering them more knowledge and competence to manage nutrition-related conditions, especially child undernutrition. In-service nutrition training interventions can help to fill the gap created by the lack of adequate nutrition training in the existing medical and nursing education system. In this way, steps can be taken toward improving the overall nutritional status of the child population.

Objective: To conduct a literature review about the ten steps of the Baby Friendly Hospital Initiative (BFHI). Methods: We sought documents and scientific papers published in the databases of PubMED, Medline, SciELO and LILACS. Results: We initially identified 110 references about the BFHI, between the years of 1979 to 2009. Approximately 21% were published in the 1990s and 79% between 2000 and 2009; 10.8% were published in books and official documents of the Ministry of Health, Pan American Health Organization and World Health Organization; and, 89.2% were in articles indexed in the consulted databases. Of these, 35 references were selected. The analyzed studies showed that changes in hospital practices according to the Ten Steps of BFHI increased the prevalence of breastfeeding. Conclusion: Through the studies analyzed, the BFHI showed effectiveness in increasing breastfeeding in many regions of the world, contributing to the reduction of infant morbidity and mortality. Los estudios analizados evidenciaron que los cambios en las prácticas hospitalarias de acuerdo con los Diez Pasos de la IHAN aumentaron la prevalencia de la lactancia materna. Conclusión: Por medio de los estudios analizados la IHAN se ha mostrado efectiva en el aumento de la práctica del amamantamiento en muchas regiones del mundo, contribuyendo con la reducción de la morbimortalidad infantil. O objetivo foi avaliar o impacto de atualização de profissionais de saúde em alimentação infantil na prevalência de anemia nos primeiros dois anos de vida. Investigou-se também a frequência da prescrição de suplementação de ferro (SF) e os fatores associados a essa prática. Conduziu-se estudo de campo randomizado por conglomerado em Unidades de Saúde (US) em uma cidade do sul do Brasil. Os profissionais das US-intervenção participaram da capacitação sobre o guia da alimentação saudável para as crianças menores de 2 anos do Ministério da Saúde. Avaliaram-se 533 crianças com idade de 12-15meses. A prevalência de anemia foi 55,7%, sem diferença significativa nos grupos intervenção (GI) e controle (GC) (p=0,34). A prevalência de prescrição do SF (84,2% vs 81,2%, p=0,40) e da adesão materna ao uso SF >180 dias (36,2% vs 35,2%; p=1,00) foi semelhante nos GI e GC. O tempo de uso do SF >180 dias foi associado à menor prevalência de anemia (46,6% vs 56,4%; p=0,03). O maior número de consultas na puericultura foi associado ao uso do SF por período ≥180 dias (p=0,04). A intervenção não foi efetiva em reduzir a prevalência de anemia entre as crianças, sendo necessário maior incentivo às mães para aderirem ao uso do suplemento de ferro nos primeiros dois anos.
