Indoor residual spraying for kala-azar vector control in Bangladesh: A continuing challenge

Abstract: BackgroundVisceral leishmaniasis (VL) in the Indian subcontinent is a fatal disease if left untreated. Between 1994 to 2013, the Ministry of Health of Bangladesh reported 1,09,266 cases of VL and 329 VL related deaths in 37 endemic districts. Indoor residual spraying (IRS) using dichlorodiphenyltrichloroethane (DDT) was used by the national programme in the 1960s to control malaria. Despite findings of research trials demonstrating that the synthetic pyrethroid deltamethrin 5 WP was very effective at reducing … Show more

“…1 ). The other district, Bandarban, was selected as it is a malaria-endemic region, and as preventative measures deltamethrin-impregnated long-lasting insecticidal nets (LLINs) are regularly distributed among the population and there is seasonal sporadic indoor residual spraying (IRS) [ 33 , 34 ]. Since the majority of Aedes -borne arboviral infections are reported from Dhaka, eight areas within Dhaka City were selected for sampling [ 35 ].…”

“…The failure in achieving Kala-azar elimination target 2017 in India may be due to irregularities in quality and frequency of DDT IRS 7 . Moreover, the same factor may be also implicated for gradual development of insecticide resistance 8 . DDT resistance not only impaired the IRS program but has also raised the possibility of resistance, phenotypic & genotypic fixation in the sand flies.…”

“…It later appeared that one of the batches of Miltefosine supplied by the national programme was a fake drug with no active substance [31], so we may consider that the effect of case management was minimal during that period. Similarly, no sand fly control activities were conducted by the government up to early 2012 [8, 12] since banned of DDT in 1997 [32], as the registration process of deltamethrin for sand fly control took a long time [13]. However, in 2013 the national programme distributed two pieces of LLIN to each patient who had completed VL treatment between 2009 and 2011 [8].…”

“…Operationally IRS is a more challenging and also more expensive method than ITN distribution. Studies in Bangladesh, India and Nepal identified that the impact of IRS is sub-optimal when it was carried out by the national programme [13, 16]. Furthermore, VL cases are sharply reducing in the countries so that it will not be sustainable to continue blanket IRS operations in all endemic sub-districts in the country.…”
