Supraglottic stenosis is a rare symptom, particularly in fibroinflammatory multifocal diseases, such as IgG4-related disease (IgG4-RD). There is still an inconsistency in the diagnosis of less-common locations of IgG4-RD, which causes a delay in the diagnosis and treatment. Our paper aims to analyze different aspects of IgG4-RD presenting as supraglottic stenosis, including the possible overlap with ANCA-associated vasculitis. We compare the usefulness of the recently revised ACR/EULAR and Comprehensive criteria and discuss treatment options. The review was performed according to PRISMA guidelines using the MEDLINE Pubmed and Scopus databases. The analysis includes nine papers describing supraglottic laryngeal stenosis in 13 patients. Furthermore, we present a case of a woman with ongoing supraglottic stenosis presenting with cough, temporary dyspnea and stridor as the symptoms of localized IgG4-RD. At the time of writing, the patient remains in remission while receiving treatment with cyclophosphamide and methylprednisolone. The symptoms of supraglottic localization of IgG4-RD may be severe; however, at that point, clinicians should suspect autoimmune etiology and attempt to modulate the autoimmune response instead of performing dilatation surgery—the effects of which may not result in extended intervals between interventions. The ACR/EULAR criteria show great specificity; however, when IgG4-RD is presumed, the specific treatment should be implemented.

Immunoglobulin G4–related disease is characterized by increased serum immunoglobulin G4 level, enlargement in the relevant organs and histopathologically intense storiform fibrosis, lymphoplasmacytic infiltration rich in immunoglobulin G4 positive plasma cells, and obliterative phlebitis. In this report, a patient who underwent a laryngeal biopsy with a pre-diagnosis of malignancy, but had findings consistent with immunoglobulin G4–related disease in the biopsy sample, is described. Immunoglobulin G4–related disease can be seen in very rare localizations. It should be kept in mind in differential diagnosis when tissues especially containing inflammation rich in plasma cells are encountered. Clinical, laboratory and pathological correlation is extremely important in the diagnosis of an immunoglobulin G4-related disease.

Most coronaviruses share a similar structure and infection pathway; thus, one can expect the similar infection mechanisms for SARS-CoV-2. 4 Coronaviruses could invade the brain via the cribriform plate close to the olfactory bulb and the olfactory epithelium. We could expect some structural changes in the olfactory bulb that were not observed in this case. However, it is important to assess the volume of the olfactory bulb that might be, at this this stage, too subtle to be detected on MRI results. Moreover, Yao et al 5 have reported that the volume of the olfactory bulb is decreased in patients with postinfectious olfactory loss and is inversely related to the duration of olfactory loss.Recently Ligget et al 6 described an expression of the olfactory receptor family on central cortical neurons, vascular smooth muscle, and upper and lower airway epithelium. Because SARS-CoV-2 infects the human respiratory epithelial cells through an impairment of the angiotensin-converting enzyme 2 receptor, we believe that this olfactory receptor family could also be selectively impaired. 1 To our knowledge, this is the first report of a patient with COVID-19 presenting essentially with an olfactory function loss. As reported by the French Society of ENT (https://www.snorl. org/category-acces-libre/alerte-anosmie-covid-19-20-mars-2020/), we believe that the association of a sudden and complete olfactory function loss, without nasal obstruction in a patient with other symptoms, such as cough or fever, should alert the clinician to suspect SARS-CoV-2 infection.
