Individual Variability in Reaction Time and Prediction of Clinical Response to Methylphenidate in Adult ADHD: A Prospective Open Label Study Using Conners’ Continuous Performance Test II

Abstract: Objective: The aim of this study was to examine whether reaction time parameters in adult patients with ADHD could predict their response to methylphenidate (MPH). Method: Previously unmedicated patients ( N = 123) were administered the Conners’ Continuous Performance Test II (CPT II) at baseline and after 6 weeks of treatment with immediate-release MPH. In addition to traditional CPT measures, we extracted intraindividual raw data and analyzed time series using linear and nonlinear mathematical models. Result… Show more

“…A high proportion of adolescents with ADHD maintain the syndrome symptoms in adulthood [ 1 ]. In addition, a significant proportion of ADHD patients are first diagnosed as adults, indicating that the developmental trajectory of this syndrome is variable [ 4 ]. ADHD in adulthood is associated with adverse occupational outcomes, impaired somatic and psychiatric health, as well as high costs for social and health services [ 4 , 5 ].…”

“…The CCPT has also been utilized as a pre-post treatment outcome indicator (Barrilleaux & Advokat, 2009; Bron et al, 2014; Fredriksen et al, 2021; Rucklidge et al, 2011; Tenenbaum et al, 2002). Bron et al (2014) conducted a 6-week, randomized, placebo-controlled cross-over study where both groups received a stimulant medication and a placebo at different intervals.…”

“…Reaction time and reaction time variability, measured as the standard deviation of the reaction time, across a CPT session are sensitive measures of overall performance and increased variability is correlated with impaired performance and treatment response (Fredriksen, Egeland, Haavik, & Fasmer, 2021; Levy, Pipingas, Harris, Farrow, & Silberstein, 2018). To our knowledge, reaction time variability has not been reported in previous studies utilizing the rCPT.…”

“…Sustained attention, the ability to focus on tasks over extended periods of time, is a fundamental cognitive domain and it is impaired in many neuropsychiatric disorders (Huntley, Hampshire, Bor, Owen, & Howard, 2017; J. Liu et al, 2021; S.…”
