Indirect Reduction of the Radial Head in Children With Chronic Monteggia Lesions

Abstract: Radial head reduction was achieved and maintained in 8 of 10 cases after primary or secondary surgery. The radial head was mildly subluxated in one case and dislocated in another case at final follow-up. ALR was performed in only 2 cases. Open reduction and ALR is not required in every case, and its need should depend on intraoperative stability of radial head.

“…The majority of authors had used triceps fascia graft (n = 8) [10,16,17,19,20,24,28,31], followed by triceps tendon (n = 6) [9,11,12,22,25,32], annular ligament remnant (n = 5) [15,18,22,28,34], and forearm fascia (n = 4) [18,30,35,36]. “…There were several reports about successful reduction of the radial head by ulnar osteotomy without open reduction in missed Monteggia fracture 1719202122. However, most reports recommended open reduction if satisfactory closed reduction was not achieved 192021.…”

“…However, most reports recommended open reduction if satisfactory closed reduction was not achieved 192021. We think that it may be difficult to reduce the radial head with ulnar osteotomy alone without open reduction, because a dislocated or torn annular ligament could form meniscoid scar tissue.…”
