Indications of Pediatric Tonsillectomy

Abstract: Possible indications for tonsillectomy include sleep apnea and other obstructive sleep-related breathing disorders, recurrent tonsillitis, peritonsillar abscess, periodic fever, aphthous stomatitis, pharyngitis, adenitis (PFAPA), and other miscellaneous rare conditions. Over the last century indications have changed, with a decrease in infectious causes and an increase in sleep apnea disorders. Sleep apnea in children is difficult to diagnose short of polysomnography (PSG) which is expensive and disturbing, es… Show more

“…The main objective of this study was to characterize the expression of PD-1 and PD-1L on B and T lymphocytes as well as the frequency of B and T cells in tonsillar tissue during chronic antigenic stimulation accompanying chronic or recurrent tonsillitis. The tonsillitis in most of cases is a result of viral pharyngitis, and approximately 15 % of the cases are evoked by bacterial infection with β-hemolytic streptococci [ 1 ]. The tonsillectomy is a routine procedure to treat hypertrophied tonsils, and it is one of the most often performed surgeries during childhood [ 11 ].…”

“…Recurrent viral and bacterial infections are the major underlying causes of tonsillitis, which typically is a self-limiting localized inflammation of the oropharynx. These infections upon continuous antigenic stimulation could induce histomorphological and functional changes in the tonsils, making tonsillectomy necessary [ 1 ]. The tonsils play a key role in host defense against invading antigens of the upper respiratory tract being a site of residence of the intraepithelial immunoglobulin-producing B cells that represent approximately of 50 % cells [ 2 ].…”

“…Tonsil surgery is common in the pediatric population, with the most common indication being sleep‐disordered breathing (SDB) due to tonsil‐related upper airway obstruction . Traditionally, tonsillectomy (TE) (complete removal of the tonsils) with or without adenoidectomy (A) (TE±A) has been the tonsil surgery of choice for this indication .…”

“…At the beginning of the twentieth century, in the preantibiotic era, infection or recurrent tonsillitis was the main indication for tonsil surgery 9. Following the introduction of antibiotics in the 1950s, the number of TEs decreased dramatically over the following decades 10…”
