Background: To present a new idea for rapid management of tooth avulsion in young children and adolescents (age: 6 to 16) by this case report. Case presentation: A nine-year-old boy presented with loss of upper permanent central incisor in early mixed dentition (6 to 10 years, phase I).The not yet fully resorbed primary canine was used as tooth transplant for the lost incisor. The intentional renounce on endodontic treatment of the primary transplant permits natural exfoliation of the transplant occurring either spontaneously or due to undermining resorption through the adjacent erupting teeth. In the late mixed dentition (10 to 16 years, phase II), the primary tooth transplant is electively removed and replaced by a premolar autotransplant for long-term rehabilitation of the meanwhile adolescent patient. Results: Primary canine autoTX acts as a temporary denture with instant surgical gap closure. Ensuing premolar autoTX in phase II then acts as permanent denture with excellent 10 years survival rates. Patient satisfaction issued by a questionnaire resulted in good to very good school grades for both phases of autotransplantation. Conclusions: The "TPTX" concept is a new surgical approach that immediately restores the patient's function and aesthetics after tooth loss in early childhood supporting the growth of local bone and soft tissue in the growing adolescent jaw.

Background: Autotransplantation is a surgical movement to reposition teeth that have been erupted, partially erupted or not yet erupted from one place to another in the same individual, either on post-extraction sockets or artificial sockets made by surgery. Dental folicle from impacted teeth may develope dentigerous cysts that will result in destruction of the jaw bone. There are many factors that influence the success of autotransplantation, one of which is adequate bone support.The purpose of this poster is to present the successful autotransplantation treatment of impacted right maxillary incisor teeth associated with dentigerous cyst and bone support replacement after cyst enucleation using a carbonate apatite collagen graft, GAMACHA®, at RSUP Dr. Sardjito. Case management: A 9-year-old girl, referred to the Oral and Maxillofacial Surgery Department of RSUP Dr. Sardjito with the complaint of right deciduous maxillary incisor persistence. Radiological examination showing persistent of 51 and 52 radices and impaction of teeth 11 and 12 in a horizontal position above radix 53. Also visible radiolucent image with a firm limit around the crown of 11 that supports the image of cyst. This patient performed cyst enucleation under general anesthesia, removal of impacted 11 and 21, followed by an artificial socket drilling at the alveolar bone that could support all aspects of the root wall although only on the apical part of the tooth root for teeth 11 and 12 placement. Autotransplantation of teeth11 and 12 followed by the application of GAMACHA® and wire fixation, strengthen with acrylic splint. Conclusion: Two years follow up after surgery, patient have no complaints either from aesthetic or functional and the result of clinical and radiological examination did not found existence of any pathological abnormalities around teeth 11 and 12.

Apesar de os traumatismos dentários serem frequentes na população infantil, os estudos disponíveis mostram IntroduçãoSegundo a classificação de traumatismos dentários da Organização Mundial de Saúde, a avulsão dentária é uma lesão grave que consiste na completa exarticulação do dente do seu alvéolo 1 . É a lesão dentoalveolar que mais causa danos funcionais, estéticos e psicológicos ao paciente, constituindo-se em uma grave urgência odontológica 2 . Ocorre principalmente em crianças [3][4][5][6][7] , com prevalência estimada de 1% a 16% 8 . As consequências de um traumatismo dentário são um problema para a odontologia 4 e o sucesso do tratamento da avulsão dentária depende basicamente de como a assistência de emergência é fornecida. O reimplante imediato é o tratamento mais apropriado para dentes permanentes traumaticamente avulsionados na tentativa de se conseguir um bom prognóstico, devendo então, ser realizado imediatamente após o acidente, se possível, ainda no local. A taxa de sobrevivência de um dente reimplantado está associada à preservação da vitalidade do seu ligamento periodontal, que por sua vez está diretamente relacionada a danos ou ao manejo inadequado da raiz (pela vítima ou pela pessoa que dá os primeiros socorros), ao tempo extra-alveolar, ao meio de armazenamento em que o dente é mantido até o reimplante (no caso de reimplante tardio) e ao tipo de reabsorção desenvolvida durante o processo de cicatrização 7,[9][10][11][12] .

Abstract: Auto-transplantation provides an excellent outcome in a growing child with the advantage that it is a biologically compatible method of tooth replacement, which promotes pulp and periodontal healing and enables orthodontic movement if necessary.
