Indications for Blood Transfusion in Pregnancy With Sickle Cell Disease

Abstract: In order to assess the value of prophylactic blood transfusion in the management of pregnant patients with sickle cell disease, a study was conducted on 571 sickle cell disease pregnancies. These patients were divided into 2 groups, one with prophylactic transfusion and the other with restricted transfusion. In spite of the reduction in the incidence of blood transfusion the rate of preterm labour, intrauterine growth retardation, perinatal mortality and maternal mortality were similar in both groups. A restri… Show more

“…3 As this systematic review demonstrates, maternal mortality in a majority of women with SCD occurred in the setting of vaso-occlusive pain episodes, 14,18,28 with pulmonary embolism and acute chest syndrome accounting for 2 additional cases each. 14,28 The positive impact of prophylactic transfusion on maternal morbidity may thus be the consequence of reducing the frequency of vaso-occlusive pain episodes.…”

“…For Koshy et al [23], systematic transfusion would be appreciated prior to 28 weeks of amenorrhea for patients with a history of perinatal mortality, preeclampsia, acute chest syndrome, stroke or severe anemia; to reduce the risk of infection and alloimmunization, which complicate subsequent transfusions. But for El Shafei et al [25], systematic transfusion cannot improve the obstetric prognosis.…”

“…The literature is rather contradictory regarding the effectiveness of exchange transfusion on maternal and fetal morbidity. Some authors found that routine transfusion does not improve obstetric outcomes [9] [10]. They do advocate this practice if there are particular medical history or in certain clinical settings.…”
