Increasing the Elective Endovascular to Open Repair Ratio of Popliteal Artery Aneurysm

Abstract: The epidemiology of elective repair of PAA has changed toward increased ER, although ER showed a higher rate of major amputations at 1 year. Confounding was considerable and a randomized trial is needed for evaluation of the best therapeutic option.

“…Amputation rates were reported in 10 of the included studies, 13,15,16,18,21,23,24,26 -28 revealing no significant differences in these rates between the OR and EVR groups (OR = 1.01; 95% CI: 0.55-1.85; P = .98).…”

“…Duration of hospitalization was reported in 12 of the included studies. 12,14,16,17,19,20,22,24,25,26 -28 An analysis of these studies indicated that EVR treatment was associated with a significant reduction in hospitalization duration relative to OR (mean difference = −3.71 days; 95%CI: −3.86 to −3.56; P < .000001). High heterogeneity was observed for the included studies with respect to this outcome ( I 2 = 94%).…”

“…An increasing proportion of popliteal artery aneurysms are found and operated because of screening for AAA. 9 These scenarios have not been taken into account in the debate about the benefits and harms of AAA screening.…”

“…1 In elective cases, open or endovascular treatment is often used when the aneurysm reaches 2 cm in diameter. 2 Results of endovascular treatment were initially disappointing, likely due to the materials used in the first generation of devices, 3 and while it continues to demonstrate lower patency rates compared with open repair using prosthetic materials or autologous vein, 4 endovascular PAA repair is now indicated in selected cases. 5 In particular, it is often used in the absence of arterial tortuosity and off-target area dilatation and when treating patients with at least 2 runoff vessels and proximal and distal stent-graft landing zones of at least 15 mm.…”
