Increasing normal–appearing grey and white matter magnetisation transfer ratio abnormality in early relapsing–remitting multiple sclerosis

Abstract: Abnormalities within normal-appearing grey and white matter (NAGM and NAWM) occur early in the clinical course of multiple sclerosis (MS) and can be detected in-vivo using the magnetisation transfer ratio (MTR). To better characterize the rates of change in both tissues and to ascertain when such changes begin, we serially studied a cohort of minimally disabled, early relapsing-remitting MS patients, using NAGM and NAWM MTR histograms. Twenty-three patients with clinically definite early relapsing-remitting MS… Show more

“…Reduced GM MTR seems to be present in all MS clinical phenotypes, starting from the earliest clinical stage of the disease. [45][46][47] Such GM abnormalities, however, increase with disease duration; for example, they were found to be more pronounced in patients with PPMS or SPMS. 31 The distribution of MTR abnormalities in the different brain GM structures still needs to be defined.…”

“…39,40 Such pathological abnormalities modify the relative proportions of mobile and immobile protons of the diseased tissue, and so it is not surprising that MT-MRI is able to show microscopic damage in NAWM that conventional imaging fails to detect. [25][26][27] Decreased MTR values have been found, using ROI and histogram analysis, in normal-appearing brain tissue (NABT) and NAWM of MS patients, 24,29,30,[41][42][43][44][45][46][47] which can precede new lesion formation. 48 -51 NABT and NAWM MTR abnormalities have been detected in patients with CIS suggestive of MS, 29,44,46 increase progressively in early RRMS 45 and early PPMS, 47 and are more pronounced in SPMS and PPMS patients than in patients with the other main disease phenotypes.…”

“…56,57 Tozer et al 58 estimated GLCM textural features extracted from magnetization transfer MR images acquired from 23 healthy controls, as well as from 32 patients with RRMS, 3 patients with SPMS, and 38 patients with CIS, and investigated the relationship between textural features and the Expanded Disability Status Scale. While the authors found no differences between the features extracted from controls and patients with CIS, several features differed between patients with MS and the 2 other groups, especially in GM (but also in WM).…”

“…Some studies found no differences between patients with MS and healthy controls, [8][9][10] whereas others suggested that thalamic involvement is detectable in patients with MS. 5,[11][12][13][14][15][16][17] The time course of thalamic involvement in MS is still an unanswered question. Although some studies have suggested that thalamic involvement starts early, 14,18,19 others have only identified damage in later stages of the disease. 15 Diffusion tensor imaging (DTI) describes the properties of water diffusion in vivo, thus disclosing important details of fiber tract orientation and providing information regarding microstructural integrity of the tissue.…”
