Klebsiella infection is often the leading cause of morbidity and mortality. Resistance pattern of antimicrobial susceptibility to commonly prescribed drugs were studied in Klebsiella isolates from a hospital in Madinah, Saudi Arabia. Clinical samples were collected from 6840 patients and screened for Klebsiella species over a period of 14 months. The strains were identified using microbiological and biochemical tests while the antibiotic susceptibility was performed by disk diffusion assay. Of all the positive samples isolated, only 275 samples were identified as Klebsiella species. Of these 66% were from males indicating that females are less vulnerable. Maximum isolates (66 to 70%) were collected from sputum and wound swabs of males. About 90% species were isolated from wound, sputum and catheter tips swabs. Antimicrobial sensitivity was studied using seventeen different antibiotics. Results revealed that antibiotic Imipenem has the highest sensitivity of 99.5% while Ampicillin revealed 100% resistance. The prescription of Imipenem antibiotic is recommended for the treatment of Klebsiella infections. In case of resistance to Imipenem, other antibiotics mainly Ceftazidime, Aztreonam, Ciprofloxacin, Cefoxitin and Piperacilline may be recommended. In acute cases, use of combined antimicrobial therapy may be required. Results also indicate that the intensity of Klebsiella infections is higher during summers (46.5%) than in winters (27.2%), while autumn (13.9%) and spring seasons (12.5%) showed the least recorded percentage. Our study will help in persistent and continuous monitoring of antimicrobial susceptibility and supplement the already available data on prevalence of antimicrobial resistance patterns of Klebsiella.

P. aeruginosa and E. coli are resistant to many antibiotics, so the treatment of infections of these pathogens has become difficult. Therefore, searching for the presence of antibiotic resistance genes in clinical isolates is of great importance. The purpose of this study is to investigate the presence of beta-lactam resistance genes and class 1 integron in multi-drug resistant P. aeruginosa and E. coli clinical strains. Vitek 2 Compact automatization system were used for identification and antibiogram of 2 P. aeruginosa and 2 E. coli isolates which were isolated from blood, urine and sputum specimens of patients whom were hospitalized in Gümüşhane State Hospital intensive care unit. Total DNA isolation was done by boiling DNA method. PCR were performed using primers of class 1 integron gene cassette, bla VIM , bla NDM, bla IMP , bla GES , bla CTXM-1 , bla CTXM-2 , bla OXA-58 , bla OXA-23 , bla OXA-51 , bla OXA-40 and bla KPC . All amplification samples were performed on 1% agarose gel and subsequently visualized under UV light. According to the results of the antibiogram, P. aeruginosa isolates showed resistance against all used antibiotics except gentamicin. The E. coli isolates were found to be resistant to cephalosporin antibiotics and susceptible to carbapenems. bla CTXM-1 -class A beta lactamase genes was found in 3 strains of 4, while no bla CTXM-2, bla GES and bla KPC genes were detected in the strains. The presence of the investigated class B and class D beta lactamase genes was not observed in any sample. The presence of class 1 integron was detected in 3 strains. According to the result of the DNA sequence analysis, it was determined that three integron positive samples had the dfrA1/AadA5 gene cassettes.Özet. P. aeruginosa ve E.coli izolatlarının birçok antibiyotiğe direnç kazanması nedeniyle bu patojenlerin neden olduğu enfeksiyonların tedavisi zorlaşmıştır. Bundan dolayı klinik izolatlarda antibiyotik direnç genlerinin belirlenmesi büyük önem arz etmektedir. Bu çalışmanın amacı çoklu-ilaç dirençli P. aeruginosa ve E.coli klinik suşlarında beta laktamaz direnç genlerinin ve sınıf 1 integronlar gen kasetlerinin varlığının araştırılmasıdır. Gümüşhane Devlet Hastanesi yoğun bakım ünitesinde yatan hastaların kan, idrar ve balgam örneklerinden izole edilen 2 P. aeruginosa ve 2 E.coli izolatlarının tanımlaması ve antibiyogramı Vitek 2 Compact otomatize sistemi ile çalışılmıştır. Total DNA izolasyonu kaynatma DNA metoduyla yapılmıştır. bla VIM , bla NDM , bla IMP , bla GES , bla CTXM-1 , bla CTXM-2 , bla OXA-58 , bla OXA-23 , bla OXA-51 , bla OXA-40 , bla KPC ve sınıf 1 integron gen kasetlerine ait primerler kullanılarak PZR'ler gerçekleştirilmiştir. Amplikasyonun gerçekleştiği tüm örnekler %1'lik agaroz jelde yürütülmüştür ve daha sonra UV ışığında görünür hale getirilmiştir. Antibiyogram sonucuna göre P. aeruginosa izolatları gentamisin hariç kullanılan tüm antibiyotiklere karşı direnç gösterdiği belirlenmiştir. Background: Treatment of Klebsiella pneumoniae (K. pneumoniae) infections has become very difficult especially in the last decades as the levels of antibiotic resistance are becoming increasingly a serious problem throughout the world. Aim: The growing number and rapid increase in carbapenems resistance which are the last resort of treatment of K. pneumoniae infections has urged for investigating some of these possible resistance mechanisms. Materials and Methods: A total of 202 K. pneumoniae isolates were selected from 593 specimens collected from patients attended 4 hospitals during the period from October 2013 to August 2014. Fifty three multi-drug resistant (MDR) K. pneumoniae isolates which showed resistance to imipenem and/or meropenem were tested phenotypically and gynotypically for the producibility of Klebsiella pneumoniae carbapenmase (KPC) and New Delhi metallo β-lactamase (NDM) and the presence of blaKPC and blaNDM genes encoding for these enzymes respectively. Results: The rate of production of both potential KPC and NDM was 35/53 (66.04%), whereas only 8 of these 53 MDR K. pneumoniae isolates (15.1%) were positive for NDM but not KPC production. The rates of detection of potential blaKPC and blaNDM genes among the 35 MDR K. pneumoniae positive isolates for the production of both KPC and NDM were 10/35 (28.57%) and 5/35 (14.28%) respectively. Conclusion: blaKPC and blaNDM were detected in some MDR isolates. However, the presence of isolates devoid of both genes suggests that resistance against carbapenems could be due to mechanisms other than the production of KPC and NDM-β-lactamases.
