Increased exercise ejection fraction and reversed remodeling after long‐term treatment with metoprolol in congestive heart failure: a randomized, stratified, double‐blind, placebo‐controlled trial in mild to moderate heart failure due to ischemic or idiopathic dilated cardiomyopathy

Abstract: Background: the effects of long-term administration of b-blockers on left ventricular (LV) function during exercise in patients with ischemic heart disease (IHD) and idiopathic dilated cardiomyopathy (DCM) are controversial. Patients and methods: patients with stable congestive heart failure (CHF) (New York heart association wNYHAx class II and III) and ejection fraction (EF) F0.40 were randomized to metoprolol, 50 mg t.i.d. or placebo for 6 months. Patients were divided into two groups: ischemic heart disease… Show more

“…␤-Blocker therapy has been shown to reduce regurgitant volumes in patients with heart failure and ischemic mitral regurgitation. 11 Surgical treatment for ischemic mitral regurgitation generally combines mitral valvuloplasty or replacement with coronary artery bypass grafting. In such patients, operative mortality is generally better after surgical repair rather than replacement of the mitral valve.…”

“…Reverse remodelling (as measured by change in LV end systolic volume) has also been reported in the Carvedilol PostInfarct Survival Control in Left Ventricular Dysfunction (CAPRICORN) study with carvedilol plus an ACE-inhibitor, but not with placebo plus an ACE-inhibitor. 53 Reverse remodelling has also been demonstrated with metoprolol 54 and bisoprolol. 55 For example, in a study in 201 patients with stable CHF, after 3 months of treatment with bisoprolol, mean LVEF had increased from 31 to 41% (P , 0.0001).…”

“…Metoprolol and carvedilol can attenuate LV remodeling. [163][164][165] In a double-blind, placebo-controlled study in 394 patients with persistent AF who underwent cardioversion, metoprolol CR/XL was effective in preventing relapse into atrial fibrillation or flutter. 166 …”
