Increased estradiol levels in venous occlusive disorder: a possible functional mechanism of venous leakage

Abstract: Venous insufficiency of the corpora cavernosa is the second most common cause of erectile dysfunction (ED). A functional insufficiency of the venous system has been hypothesised, but the cause is still unclear. To evaluate a possible endocrine mechanism, we have studied hormone profile in a group of nine patients with pure venous-leakage (VL) compared with a control group of 15 patients with ED of different origin. Prolactin, testosterone and gonadotropin levels did not differ among the two groups, while estra… Show more

“…High estradiol values are considered detrimental to male sexual function. Estradiol values have been noted to be significantly higher in ED patients with venous leak than in controls, supporting the hypothesis that estradiol level can adversely influence penile smooth muscle function [120].…”

“…Early detumescence, clinically called venous leakage, is a common presenting symptom in ED, especially when androgen deficiency is present, but Mancini et al. [110] suggested that increased estradiol levels may also contribute to ED. The authors pointed out that both ED and estradiol levels appear to increase with age.…”

“…However, even in this setting, the absence of estradiol effect on LH secretion is still puzzling. Thus, taking into account that high estradiol levels have already been described as the only abnormality in a subset of patients with ED, the hypothesis that the later might not only be caused by androgen deficiency is becoming increasingly evident [8, 20]. Furthermore, it has been reported that the chronic exposure to phosphodiesterase type 5 inhibitors (PDE5i), widely used for the treatment of ED, may influence serum estradiol levels [33, 34].…”

“…One of the most plausible mechanisms by which obesity contributes to T levels decline is the adipose tissue dependent aromatization of T to estradiol [15]. In line with these remarks, hormonal ED etiology might not be confined to androgen deficiency, as isolated high estradiol levels have been reported in some ED patients [8, 20]. …”
