Increased epithelial permeability in pulmonary fibrosis in relation to disease progression

Abstract: Epithelial injury contributes to pathogenesis in idiopathic pulmonary fibrosis (IPF) but its role in the interstitial lung disease (ILD) of systemic sclerosis (SSc) is uncertain. We quantified the prognostic significance of inhaled technetium-99m ( 99m Tc)-labelled diethylene triamine pentacetate (DTPA) pulmonary clearance, a marker of the extent of epithelial injury, in both diseases. Baseline 99mTc-DTPA pulmonary clearance was evaluated retrospectively in patients with SSc-ILD (n5168) and IPF (n597) against … Show more

“…In this regard, a staging system using disease extent on HRCT with an ancillary role for the lung function test in ''indeterminate'' cases [104,105], and a series of markers of epithelial injury and epithelial cell damage have been implemented [106][107][108][109]. Recently, increased levels of IL-6 have been proposed as marker of a decline in FVC and/or DLCO within the first year, and of death within the first 30 months in patients with mild disease [110].…”

“…The increased clearance of technetium-labeled diethylene triamine pentaacetate (99mTc-DTPA) aerosol from the lungs of patients with SSc-ILD is indicative of a breached epithelial barrier and is associated with more rapid decline in FVC [20, 21], independently of disease severity. Lung epithelium-specific proteins leaking into the circulation may thus represent potential biomarkers of alveolar epithelial damage.…”

“…Abnormal angiogenesis has furthermore been linked to the development of fibrotic disorders of the lung [7]. Together, these changes result in an increase in the permeability of the alveolar capillary barrier, which can be detected clinically by increased diethylene triamine penta-acetic acid clearance [8]. Increased alveolar capillary barrier permeability may also be associated with early mortality in IPF [9].…”
