Introduction: Recent advances in three-dimensional (3D) printing technology have made it possible to print surgical devices. We report our initial experience with the printing and deployment of endoscopic and laparoscopic equipment. Materials and Methods: We created computer-aided designs for ureteral stents and laparoscopic trocars using SolidWorks. We developed three generations of stents, which were printed with an Objet500 Connex printer, and a fourth generation was printed with an EOSINT P395 printer. The trocars were printed with an Objet30 Pro printer. We deployed the printed stents and trocars in a female cadaver and in vivo porcine model. We compared the printed trocars to two standard trocars for defect area and length using a digital caliper. Paired T-tests and ANOVA were used to test for statistical difference. Results: The first two generations of stents (7F and 9F) were functional failures as their diminutive inner lumen failed to allow the passage of a 0.035 guidewire. The third generation 12F stent allowed passage of a 0.035 guidewire. The 12F diameter limited its deployment, but it was introduced in a cadaver through a ureteral access sheath. The fourth-generation 9F stents were printed and deployed in a porcine model using the standard Seldinger technique. The printed trocars were functional for the maintenance of the pneumoperitoneum and instrument passage. The printed trocars had larger superficial defect areas ( p < 0.001) and lengths ( p = 0.001) compared to Karl Storz and Ethicon trocars (29.41, 18.06, and 17.22 mm

Non-bladed trocars, radially-dilating systems, and conical blunt devices are considered less traumatic to the abdominal wall because they do not incise the fascia itself. Consequently, several authors have suggested that closure of the abdominal fascia may be unnecessary if such non-bladed laparoscopic trocars are used. We report of a case in whom a port-site hernia was diagnosed at the site of a 12-mm non-bladed trocar 11 days after laparoscopic nephrectomy.

Zugangsbedingte Komplikationen stellen einen großen Teil der Komplikationen dar, die im Rahmen laparoskopischer Operationen beobachtet werden. Einige können letale Folgen haben, insbesondere wenn sie nicht sofort erkannt und adäquat therapiert werden. Bei korrekter Handhabung der Instrumente und Geräte und bei Beachtung wichtiger Grundregeln können ernsthafte Komplikationen reduziert werden. Im Mittelpunkt stehen die Risiken und Probleme beim Einbringen des ersten Trokares, da dies in der Regel ohne visuelle Beobachtung der intraabdominellen Strukturen geschieht. Beim Zugang wird die geschlossene von der direkten und der offenen Technik unterschieden. Das geschlossene Eingehen mit Anlage eines Pneumoperitoneums über eine Veress-Nadel stellt das derzeit noch am häufigsten angewandte Verfahren dar. Bei Narben nach vorausgegangenen Laparotomien im Bereich der geplanten Trokarinzision muss eine Lokalisation außerhalb des Narbenareales oder der offene Zugang mit Durchführung einer Minilaparotomie gewählt werden.Auf dem Markt werden verschiedenste Trokarsysteme angeboten, die unterschiedliche Vor-und Nachteile aufweisen. Eine evidenzbasierte Bewertung der verschiedenen Zugangstechniken und Trokarsysteme, die auf kontrollierten Studien basiert, ist derzeit noch nicht möglich. Neben der weiteren Verbesserung der Trokarsysteme stellt die adäquate Schulung die wichtigste Voraussetzung zur weiteren Reduktion zugangsbedingter Komplikationen dar. Insbesondere die anatomischen Grundlagen und eine standardisierte chirurgische Technik müssen strukturiert vermittelt werden.Weiterhin sind die kontinuierliche Dokumentation zugangsbedingter Komplikationen und eine exakte Fehleranalyse erforderlich, um Risikofaktoren zu identifizieren, aus den Erfahrungen zu lernen und eine entsprechende Anpassung des Vorgehens zu ermöglichen. Zugangsbedingte Komplikationen werden in der Literatur mit einer Inzidenz zwischen 0,2 und 3,1% angegeben [11,21,25,28,49,51,60]. Nicht nur die relative Häufig-keit zugangsbedingter Komplikationen, sondern insbesondere die potenziell vital bedrohlichen Folgen sind der Anlass, das Thema an dieser Stelle vorzustellen. Der Beitrag vermittelt einen Überblick über die Palette der zugangsbedingten Komplikationen bei laparoskopischen Operationen und wichtige Regeln und Tipps zu deren Vermeidung. In diesem Zusammenhang werden zunächst die derzeit am häufigsten verwendeten Zugangstechniken sowie Kanülen und Trokarsysteme vorgestellt.
