Incidental Durotomy Following Transforaminal Lumbar Interbody Fusion Performed with the Modified Wiltse Approach

Abstract: Purpose Incidental durotomy is an intraoperative complication that occurs in 3 to 27% of lumbar spine surgeries. It has been reported more commonly following revision spinal procedures. Study Objectives To investigate the frequency of incidental durotomy while performing transforaminal lumbar interbody fusion (TLIF) using the modified Wiltse approach. A secondary goal was to compare the incidence of durotomy in patients undergoing primary spine surgery with those undergoing revision surgery. … Show more

“…1,6,15,16 We included unintentional (n = 140), traumatic (n = 29), and intentional (n = 1) durotomies, whereas most previous studies include only unintentional durotomies. 1-4,6,10-12,15,17,18 This may explain our high durotomy incidence. In addition, our academic institution includes trainees who may be more likely to cause durotomies.…”

“…Durotomies occur during 2% to 20% of spine surgeries. 1-3 Persistent cerebrospinal fluid (CSF) egress may amplify postoperative complications. 1,4 Specifically, persistent postoperative CSF egress imposes elevated incidence of positional headaches, durocutaneous fistulas, wound infection, 5 delayed wound healing, 6 intracranial hemorrhage, 7,8 nerve root entrapment, 9 meningitis, 5 pseudoarthrosis, 4 and other serious adverse events.…”
