Incidental Benign Metastasizing Leiomyoma in a Patient with Bone Sarcoma: A Case Report

Abstract: Background. The benign metastasizing leiomyoma is an exceptionally rare entity; it presents with ectopic leiomyoma nodules with a benign pattern. Symptoms vary according to the anatomic location. The diagnosis is histopathological, usually in patients with history of hysterectomy. Case Presentation. A 36-year-old female with 2-month history of left knee pain was diagnosed with bone fibrosarcoma. A CT scan showed pulmonary nodules. The patient started neoadjuvant chemotherapy. “…In review study, microscopic examination of haematoxylin and eosin slides has demonstrated the characteristic features of smooth muscle cell differentiation, which was also confirmed by immunohistochemistry smooth muscle actin positivity.Additional immunohistochemistry Ki67 showed a low tumour cell proliferation index, which favors a benign behavior [ 3 , 4 , 7 , 8 , 11 14 , 17 , 18 , 20 22 , 27 , 31 37 , 41 55 ].…”

“…In the case we described, the myomectomy was the primary procedure – similar as in most other cases of BML. Other procedures, as described in articles, included a total or subtotal hysterectomy [ 9 , 15 19 ]. There were only 3 cases described where female patients were diagnosed with BML without any prior gynecological surgery [ 1 , 9 , 20 ].…”

“…The second suggests that BML represents a low grade leiomyosarcoma. The third advocates a hematogenous dissemination of the primary lesion [ 9 , 15 18 ].…”

“…The clinical presentation of BML depends largely on its site of involvement, of which the lung is most frequent. BML involving the lung is usually asymptomatic and found incidentally on imaging, such as during staging workup for unrelated malignancies [ 4 ]. The nodules are usually found after hysterectomy or after myomectomy at durations of 3 months to 20 years [ 3 ].…”
