Incidence of Tuberculosis in the United States among HIV-Infected Persons

Abstract: Incidence of tuberculosis was higher in the eastern United States, in patients with CD4 counts of less than 200 cells/mm3, and in PPD-positive patients. Analysis of tuberculin reaction size supports the current interpretive criteria of the Centers for Disease Control and Prevention. Nonreactivity to mumps antigen indicated increased risk for tuberculosis independent of PPD response.

“…Conversely, in the USA and Western Europe, a decline in the incidence of TB in HIV-infected patients has been observed in the last decades; however, remarkable regional differences have been found in Europe, with rates four to seven times higher in southwest Europe than in other European regions [96,97].…”

“…Similar correlation was reported by other Indian and foreign studies. 14,15,16 Since HIV/AIDS leads to immunosuppression, it is strongest of all known risk factors for development of TB. Thus, the patients who have CD4 count <350 cells/ μl have significantly more chances of acquiring TB (p=0.001) confirming further the importance ofCD4 counts.…”

“…Attesting to the importance of CD4 T cells in host defenses during LTBI, the risk of TB reactivation increases dramatically in patients with HIV coinfection, from 5 to 10% per lifetime in immunocompetent hosts to as high as 10% annually in coinfected patients (106), and the risk of TB reactivation rises as the CD4 cell count declines (107)(108)(109)(110).…”
