Incidence of spinal deformity in children after multiple level laminectomy for selective posterior rhizotomy

Abstract: Fifty-five children with cerebral palsy had multiple-level laminectomies for selective posterior rhizotomies for the relief of spasticity. They were followed up clinically and radiologically to assess their spinal stability and the possible development of post-laminectomy deformity of the spine. The majority of the deformities found were related to cerebral palsy and did not appear to be due to the laminectomy: 16% had scoliosis, 5% kyphosis, 7% lordosis, and 9% spondylolysis/spondylolisthesis. Spondylolysis i… Show more

“…Asymptomatic spondylolysis or spondylolisthesis was seen in 9% of patients undergoing multilevel laminectomy for rhizotomy [64]. Although the incidence of spondylolysis or spondylolisthesis is higher than that in the normal population, it is not when compared with the reported incidence of 21% in spastic diplegic cerebral palsy [65].…”

“…Although the incidence of spondylolysis or spondylolisthesis is higher than that in the normal population, it is not when compared with the reported incidence of 21% in spastic diplegic cerebral palsy [65]. Scoliosis, kyphosis or lordosis was no more prevalent in rhizotomy patients than in cerebral palsy patients in general [64]. Spinal stenosis was seen in 1.5% of patients after multilevel laminectomy or laminotomy for dorsal rhizotomy [66].…”

“…Since the report of Raimondi et al [19] in 1976. a number of authors have recommended replacement lami noplasty [20][21][22], Its value in the prevention of delayed deformities and possible protection against the develop ment of musculoskeletal pain has been reported [21,22]. In this particular group of patients, the author believes that the main value of this procedure could be the restora tion of the spinal anatomy, therefore, rendering future surgery in the event of recurrence 'safer'.…”

“…Previous reports on multiple level lumbar laminectomies in children did not show an in creased incidence in spinal instability or deformity [25]. The follow-up on the spinal condition of children in the Capetown series revealed no increased incidence o f defor mity beyond that normally seen in children with cerebral palsy [26). The restoration of the normal bony anatomy may be beneficial however, for improved muscle attach ment.…”
