Incidence of potentially disruptive medical and social events in older adults with and without dementia

Abstract: Background Potentially disruptive medical, surgical, and social events—such as pneumonia, hip fracture, and widowhood—may accelerate the trajectory of decline and impact caregiving needs in older adults, especially among people with dementia (PWD). Prior research has focused primarily on nursing home residents with dementia. “…Dementia is a case in point. The trajectory of incapacity in cognitive impairment and dementia, as the authors of the study in JAGS note, is often “insidious and prolonged.” 1 Even allowing for disruptive events that alter the trajectory of decline in dementia, 4 there is rarely a clear demarcation between a state of health and a state of serious illness. The same is true of patients at high risk of dementia.…”

“…The initial COVID-19 pandemic social restrictions therefore provide a unique opportunity to examine how older adults can learn and adapt to an acute stressor (i.e., shelter-in-place orders) through digital technologies. Experiences during the pandemic, in turn, can inform strategies to help older adults adapt to future disruptive health events or crises (such as decreased mobility following a hip fracture, the loss of a spouse, or a natural disaster) that may confine them to their home and/or result in sudden social isolation (Hunt et al, 2022). Existing literature on older adults' use of digital technologies has primarily focused on the uptake of individual technologies.…”
