Incidence of Immediate Gadolinium Contrast Media Reactions

Abstract: This limited retrospective analysis shows that gadolinium-based contrast agents are very safe, with only rare reports of death, and raises the possibility that nonionic linear gadolinium-based contrast agents and gadopentetate dimeglumine may have fewer severe immediate adverse events compared with gadobenate dimeglumine.

“…Our results are completely in line with the recently published data extracted from the Euro-CMR study in which 30 acute adverse reactions (0.17%) occurred in a large cohort of 17,767 doses administered [31]. Similar findings were reported in larger studies analyzing the incidence of gadolinium-related contrast media reactions reporting event rates ranging between 0.04 and 2.2% [32][33][34].…”

“…We could not analyze differences in reaction rates between the various gadolinium-based contrast molecules due to the limited number of events observed in our population although a lower adverse reactions incidence for nonionic gadolinium-based contrast agents has been reported in literature as compared to ionic linear or macrocyclic agents [32].…”

“…Administration of GBCA is associated with a small risk of anaphylaxis44 and development of nephrogenic systemic fibrosis (NSF). The latter has been associated especially with linear GBCA and renal dysfunction 45.…”

“…Severe life-threatening anaphylactic and fatal reactions do occur, but are exceedingly rare (0.001% to 0.01%) [13,[15][16][17], with only 40 deaths reported in 51 million GBCA doses administered between 2004 and 2009 [18]. The rarity of severe reactions makes it difficult to analyze accurately any differences between the reactions associated with individual GBCAs.…”

“…Jung et al [14] also identified female gender and the number of exposures to MR contrast media as risk factors. Previous allergic reaction to a GBCA [14,18] is also a risk factor. The recurrence rate of hypersensitivity-like reactions is nearly 30% [14].…”
