Incidence of Cholesteatoma with Cleft Palate

Abstract: The overall incidence of cholesteatoma in Iowa has been estimated to be 6.01/100,000 population, or less than 0.01%. However, in patients with cleft palate seen at the University of Iowa Cleft Palate Clinic between 1947 and 1968 and followed for at least 10 years, the risk of developing cholesteatoma was 9.2%. In order to determine whether this high risk of cholesteatoma is still valid, a comparable retrospective study was undertaken of 153 patients with cleft palate who were born between 1969 and 1977, were e… Show more

“…Excluding patients with cleft lip, the incidences of COM and cholesteatoma in cleft palate were 20% and 2% respectively. These figures are in broad agreement with those of Dominguez and Harker, 32 who reported an incidence of cholesteatoma among children with cleft palate in Iowa of 2.6%, and Moeller, 33 who reported an incidence in Norway of 0.9%. Our figures do not account for those children attending otolaryngology services elsewhere, some of whom may never have developed ear problems, and so may well represent a slight over‐estimate of the incidence of COM in cleft palate.…”

“…The prevalence of chronic otitis media and cholesteatoma is high among patients with cleft palate, with the reported prevalences of cholesteatoma ranging from 0.9% to 9.2% 31–33 . In the present study, 20 out of 104 patients followed up at our department developed chronic otitis media (19%), with two developing a cholesteatoma (1.9%).…”

“…This was followed by accumulation of exfoliated keratin in this pocket with resultant cholesteatoma formation. The theory was supported experimentally by formation of cholesteatoma in 75% of Mongolian gerbils following bilateral Eustachian tube obstruction by electrocautery and also by higher incidence of cholesteatoma among patients with cleft palate in whom the Eustachian tube was malfunctioning [29,30]. Recently, Jackler et al [23] criticized the invagination theory in several respects.…”

“…It has also to be noted that severe otologic complications, such as cholesteatoma, are nowadays rarely seen in palate cleft patients, which we believe can be attributed to a better awareness of the disease and to a more active treatment. Dominguez and Harker 11 reported a 9.2% risk for developing cholesteatoma at the University of Iowa Cleft Palate Clinic in palate cleft patients during the years 1947 and 1968. A comparable cohort of patients born between the years 1969 to 1977 were retrospectively reviewed, and only 2.6% of the patients developed cholesteatoma.…”
