Incidence, characteristics, and management of recently diagnosed, microscopically invasive breast cancer by receptor status: Iowa SEER 2000 to 2013

Abstract: Background Recent incidence, treatment patterns and outcomes for node negative microscopically invasive breast cancer (MIBC) have not been reported. Methods State Health Registry of Iowa data identified women with ductal carcinoma in situ (DCIS), MIBC, and Stage I breast cancer excluding MIBC (Stage 1BC). Results From 2000–2013, 1,706, 193 and 4,514 women were diagnosed with DCIS, MIBC and Stage 1BC, respectively. MIBC increased at an annual percentage change of 2.1 (p=0.041). MIBC was more frequently huma… Show more

“…In terms of pathological characteristics and prognosis, DCISMi resembles closer to stage I breast cancer rather than pure DCIS [11, 12]. Some studies found that patients with microinvasion had a worse prognosis than patients with pure DCIS [11, 13, 14].…”

“…Therefore, more rigorous systematic therapy was performed in DCISMi. The rates of mastectomy, post-lumpectomy radiation and chemotherapy were higher for DCISMi than DCIS (40.9, 91.0, 4.1% for DCISMi; 30.6, 80.6, 1.9% for DCIS respectively) [12]. Chemotherapy was not recommended in the guideline of National Comprehensive Cancer Network (NCCN) [15] and the SSO-ASTRO-ASCO DCIS Consensus, both for pure DCIS and DCISMi.…”

“…However, no national BC screening program has ever been set up in any country beyond that age. BC incidence increases with age, including for the elderly, therefore addressing the question of extending prevention guidelines to include that age group seems a priority (6)(7)(8)(9)(10).…”

“…DCISM comprises about 1% of all breast cancer cases 4,5 . Previous study showed that women are presenting with DCISM more frequently since 2000, 6 supporting that the management of DCISM is an increasingly common clinical scenario. However, the survival outcomes and optimal treatment recommendations of these patients are unclear.…”
