Incidence and Timing of Presentation of Necrotizing Enterocolitis in Preterm Infants

Abstract: Among infants <33 weeks' gestation, NEC appears to present at mean age of 7 days in more mature infants, whereas onset of NEC is delayed to 32 days of age in smaller, lower GA infants. Further studies are required to understand the etiology of this disease process.

“…It has been hypothesized that maturation of immune-competent PCs is crucial for NEC development (4,(6)(7)(8). In the preterm gut, PCs secrete defensins that might contribute to an excessive inflammatory response as observed in NEC (4).…”

“…NEC involves the preterm intestine and has a complex pathophysiology in which bacterial invasion and an excessive inflammatory response is an important contributing factor (5,6). Unfortunately, the exact pathophysiology is incompletely understood. It has been hypothesized that maturation of immune-competent PCs is crucial for NEC development (4,(6)(7)(8). In the preterm gut, PCs secrete defensins that might contribute to an excessive inflammatory response as observed in NEC (4).…”

“…These infants are predominantly extremely premature infants, 1 who most often develop injury when they reach a corrected gestation age between 28 and 32 weeks. 11,12 NEC is thought to occur in these infants when intestinal bacteria penetrate the immature intestinal epithelial barrier defenses, leading to bacterial invasion of subepithelial tissues, resulting in development of inflammation and tissue necrosis. 13,14 In contrast, the second group of infants develops spontaneous intestinal perforation (SIP), which is distinctly different from NEC.…”
