Incidence and risk factors for tenofovir-associated renal toxicity in HIV-infected patients

Abstract: Mild to moderate renal failure is a frequent complication during treatment with TDF although severe renal impairment is scarce. Risk factors include age, duration of treatment with TDF, elevated baseline creatinine levels, and treatment with protease inhibitor boosted with ritonavir combinations.

“…Renal TFV toxicity in humans is characterized by Fanconi Syndrome, and depending on the dose, development of irreversible impaired renal function. There is a positive correlation between increased TFV plasma concentration, renal toxicity and the duration that patients were treated with TFV [20,21]. Because treatment with TFV is chronic, examination of the mechanism of renal toxicity is clinically relevant.…”

“…Several reports from Japan have shown the importance of low body weight and body mass index (BMI) for developing TDF-associated renal dysfunction defined as a ≥25% decline in estimated glomerular filtration rate (eGFR) 181920. However, in studies conducted in Europe and North America, body weight or BMI was not related to the development of TDF-induced renal insufficiency, as evaluated with decreased eGFR 115172122…”

“…Against this in the study by Queseda et al, increasing age was listed amongst the risk factors 18 . Overall, among the possible causes of renal impairment prior to cART initiation in our study, HIV infection was the known common factor.…”

“…As such, TDF is a probable cause of the decline in CrCl. However, as Queseda et al, and Chaisiri et al, discuss, the plausible synergy between TDF and ritonavir (RTV) as a risk factor for renal impairment cannot be overlooked 17,18 . At the time of data collection, all these patients were receiving TDF containing second line regimens, which means that they had not experienced TDF-associated acute renal impairment such as the Fanconi Syndrome.…”
