Incidence and predictors of left atrial appendage thrombus on transesophageal echocardiography before elective cardioversion

Abstract: Guidelines recommend transesophageal echocardiography (TEE) before cardioversion in thrombogenic arrhythmias when the requirement of ≥ 3 weeks of anticoagulation is not met. Current data to support this approach, especially with direct oral anticoagulants (DOAC), are scarce. We analyzed consecutive elective pre-cardioversion TEE in a high-volume electrophysiology center for the occurrence of left atrial appendage (LAA) thrombi or reduced LAA flow velocity. Possible predictors were recorded and compared in a mu… Show more

“…Both studies report relatively high rates of LAAT including dense SEC influencing treatment strategy (10% and 11.7%, respectively). 9 , 31 In the present study, age applied as a continuous or categorical covariate in regression analysis was not an independent predictor of LAAT.…”

“…Recently, Wegner et al 24 were able to demonstrate an association of QRS width and the presence of LAA thrombus and reduced LAA flow in 512 patients receiving transoesophageal echocardiography before cardioversion. However, in this study, left ventricular ejection fraction differed between the group with normal LAA flow and LAA thrombus (55% vs. 35%).…”
