Introdução: A hérnia diafragmática traumática (HDT) é uma condição rara, resultante de lesões toracoabdominais fechadas ou penetrantes, onde há comprometimento dos músculos intercostais. A incidência varia de 0,8% a 42%, dependendo do local acometido (Yaman e Derici, 2015). A ruptura diafragmática foi descrita pela primeira vez em 1541 por Sennertus após uma autópsia e, posteriormente, por Ambroise Paré em 1579, que definiu suas fases. A primeira sutura bem-sucedida de uma HDT foi realizada por Riolfi em 1886. Com o tempo, avanços nas técnicas de diagnóstico e tratamento têm proporcionado dados mais precisos sobre a sensibilidade e especificidade dos métodos de avaliação. Materiais, sujeitos e métodos: Foram consultados artigos científicos e de revisão publicados e referenciados na SciELO entre fevereiro de 2001 e 2015. Resultados e discussão: Apesar dos avanços tecnológicos na detecção da ruptura diafragmática, a ausência de diagnóstico na fase aguda é o principal fator para o desenvolvimento de HDT crônica. A abordagem laparoscópica é fundamental no reparo dessas lesões, apresentando alta sensibilidade e especificidade, além de melhores resultados pós-operatórios quando comparada a outras opções terapêuticas. Considerações finais: A abordagem laparoscópica para a HDT crônica tem mostrado bons resultados, mas depende da disponibilidade tecnológica e da habilidade do cirurgião.
