Incidence and Mortality of Acute Renal Failure in Medicare Beneficiaries, 1992 to 2001

Abstract: This study's objective was to determine the incidence and mortality of acute renal failure (ARF) in Medicare beneficiaries. Data were from hospitalized Medicare beneficiaries (5,403,015 discharges) between 1992 and 2001 from the 5% sample of Medicare claims. For 1992 to 2001, the overall incidence rate of ARF was 23.8 cases per 1000 discharges, with rates increasing by approximately 11% per year. Older age, male gender, and black race were strongly associated (P < 0.0001) with ARF. The overall in-hospital deat… Show more

“…The large scale of our study allowed identification of novel AKI demographics in children. The incidence of AKI was highest among African Americans; although AKI has been shown to be more common among African-American adults (1,33), this study is the first study to show this pattern in children. Additionally, AKI was associated with boys.…”

“…2 Although two distinct entities, emerging evidence strongly indicates close interconnection between AKI and CKD, wherein the occurrence of one strongly predicts the risk of the other. 3,4 This interconnection also points to the presence of possible common underlying molecular mechanisms in AKI and CKD. 4 Renal tubular epithelial cells constitute most of the renal mass and are the common damaged cell type in both AKI and CKD.…”

“…8 Data also indicate that the incidence is increasing significantly, while crude mortality rates have improved only marginally. [9][10][11] These data imply that the burden of illness attributable to AKI has increased, and while mortality rates are relatively stable, these figures will still translate into a greater absolute number of deaths for patients with AKI. There are plausible explanations for these trends.…”
