Incidence and Cost of Hospital Admissions Secondary to Drug Interactions Involving Theophylline

Abstract: The incidence of hospital admissions secondary to theophylline drug interactions with cimetidine, ciprofloxacin, or erythromycin is low, but the admissions represent considerable expense, even when distributed among all patients at risk for the interactions.

“…To quantify the impact of chemotherapy-induced toxicity, the incidence method as described by Hamilton was used in the current study. 11 Briefly, the incidence method requires an estimate of the incremental cost of a particular toxic event and the percent incidence of the event in a group of patients receiving treatment with the drug in question. This cost estimate is then multiplied by the percent incidence.…”

“…Of the three agents, nabpaclitaxel had the highest drug cost per cycle which translated to the highest cost per course when the median number of cycles were incorporated into the analysis (Table 5). However, when the cost impact of side-effects was estimated through the application of the incidence method, 11 the data revealed that nab-paclitaxel would require the least amount of health care resources for the management of side-effects ($597 vs. paclitaxel at $1227 vs. docetaxel at $2626). Furthermore, it is important to point out that the quality of life benefit (i.e., in the form of a utility gain) secondary to nab-paclitaxel's improved side-effect profile was not yet applied.…”

“…For example, there are several reports of phenytoin toxicity in patients also taking cimetidine 26 . Cimetidine has also been reported to be involved in toxic drug–drug interactions with theophylline, 28 triazolam, 29 procainamide, 30 and lidocaine 31 . As a result of its high risk for potentially toxic drug–drug interactions, cimetidine must be used with caution in patients who are receiving other medications.…”

“…To measure the cost of these treatment-related toxicities, the incidence method developed by Hamilton and Gordon was used. 19 To apply this method, the incidence (%) of the event reported in a group of patients receiving the treatment is needed along with the cost to treat the event. The percent incidence is then multiplied by the cost per event.…”
