Inappropriate use of thyroid ultrasound: a systematic review and meta-analysis

Abstract: Purpose Nearly half the cases of thyroid cancer, a malignancy rapidly rising in incidence within the United States, are attributable to small and asymptomatic papillary thyroid cancers that will not increase mortality. A primary driver of thyroid cancer overdiagnosis is ultrasound use. It is therefore valuable to understand how inappropriate use of thyroid ultrasound is defined, analyze the current evidence for its frequency, and identify interventions to lessen misuse. Methods Ovid MEDLINE(R), Ovid EMBASE, an… Show more

“…It could be due to deemed unnecessary by reviewing endocrinologists, functional disease or nonspecific symptoms without specifying the presence of a palpable mass/ nodule, hypothyroidism, no history of thyroid nodule or neck irradiation. [ 27 ] In this study, there was a significant difference in the symptoms of mass in the neck of the subjects ( P = .008). Moreover, there was a strong correlation between goiter palpation dan US thyroid result ( R = 0.773, P = .00).…”

“…Additionally, revision of modern risk criteria via the proposed inclusion of strain elastography or color doppler may improve objectivity and reduce variability between radiologists 66,67 . Up to 46% of thyroid ultrasounds are ordered unnecessarily and not in agreement with Choosing Wisely recommendations 68 . Hence, an improvement in inter‐rater reliability is imperative in facilitating clinical uptake of thyroid ultrasound criteria since a lack of trust in reporting will lead to needlessly repeated ultrasounds, increased hospital expenditures, and unnecessary thyroid investigations.…”

“…Thyroid cancer is one of the most common malignancies worldwide and could become the fourth most common cancer overall by 2030. Its rising incidence is thought to relate to the rapidly expanding use of point-of-care thyroid ultrasound (22), even though this explains only part of that rise (23). Thousands of thyroid nodules are subjected to surgery based on a thyroid FNA procedure and subsequent cytology.…”
