Inappropriate Sinus Tachycardia Following Viral Illness

Abstract: A 67-year-old female patient with a past medical history of menopause, migraines, and gastro-esophageal disease presented with palpitation, fatigue, and shortness of breath. One month prior to her presentation, she reported having flu-like symptoms. Her EKG showed sinus tachycardia with no other abnormality. Laboratory findings, along with imaging, showed normal results. The event monitor failed to detect any arrythmias. We report a case of inappropriate sinus tachycardia secondary to viral infection as a diag… Show more

“…An association between viral illnesses (including SARS-CoV-2 infection) and IST has been described. 36,37 Furthermore, IST has been observed in noninfectious inflammatory diseases such as multiple sclerosis; notably, tachycardia subsided after intravenous methylprednisolone bolus. 38 The link between inflammation and IST is complex and not elucidated fully.…”

“…The ECG in IST shows increased heart rate, normal P with morphology similar to sinus rhythm, normal qrs complex and T wave with normal ST segment. 9 As treatment of IST is challenging, the initial medical treatment involves prescribing long-term β-blockers but β-blockers can sometimes be ineffective even in high doses. Ivabradine may be the another drug of choice, but it may cause excessive bradycardia.…”

“…7 IST, also called non-paroxysmal sinus tachycardia, is a clinical condition characterised by an elevated resting heart rate of more than 100 beats per minute associated with disturbing palpitations and/or minimal activity that can accelerate it quickly. 9 The palpitations and tachycardia associated with IST are very disturbing for patients. Besides tachycardia, this condition may be associated with fatigue, dizziness or poor exercise tolerance.…”
