The incidence of infections caused by multidrug-resistant Gram-positive bacteria is increasing in the past years. The pathogens such as glycopeptid-resistant enterococci, methicillin-resistant Staphylococcus aureus (MRSA), and coagulase-negative staphylococci (CoNS) have emerged widely and they contribute an increase. The treatment of the patients infected with the multidrug-resistant pathogens is extremely difficult. The aim of this study was to determine in vitro susceptibility of vancomycin-resistant enterococci (VRE) and staphylococci against linezolid that may provide new alternative to treat. Methods: A total of 80 isolates of staphylococci (30 MRSA, 30 MSSA, and 20 Methicillin-resistant coagulase-negative staphylococci [MRCoNS]) obtained from various clinical specimens and 20 VRE isolates recovered from blood and rectal swab specimens were sent to our microbiology laboratory. Linezolid susceptibility was determined by disc diffusion methods and E-test for all isolates. In addition, for all isolates, sensitivity to other antibacterials was detected by Kirby-Bauer disc diffusion method according to guidelines established by the Clinical and Laboratory Standards Institute. Results: The results showed that all strains were fully susceptible to linezolid (minimal inhibitory concentration [MIC] ≤2 µG/ml). The linezolid effectiveness is not different between the MRSA and MSSA strains. MICs were changed for MRSA from 0.018 to 2 µg/ml, MSSA from 0.25 to 1.5 µg/ml, and MRCoNS from 0.19 to 1 µg/ml. MICs were changed from 0.38 to 2 µg/ ml for VRE strains. Discussion and Conclusion: As a result of the present study, it was decided that linezolid appears to be a good alternative in the treatment of infections caused by Gram-positive bacteria, especially those resistant to glycopeptides or with reduced sensitivity.

Committee on Antimicrobial Susceptibility Testing (EUCAST)'in önerileri doğrultusunda 30 µg'lık sefoksitin diski ile yapılan Kirby-Bauer disk difüzyon yöntemiyle çalışılmıştır. Vankomisin ve linezolid için minimum inhibitör konsantrasyon (MİK) değerleri gradiyent strip difüzyon yöntemine göre belirlenmiştir. Seftarolin için MİK değerleri sıvı mikrodilüsyon yöntemi ile Clinical and Laboratory Standards Institute (CLSI)'ın önerileri doğrultusunda çalışılmıştır. Bulgular: Tüm izolatlar vankomisin (MİK<2 µg/ml) ve linezolid'e (MİK<1 µg/ml) duyarlı bulunmuştur. Seftarolin MRSA izolatlarının 45/50 (%90)'sinde duyarlı tespit edilmiştir. Seftarolinin, MRSA izolatlarında MİK 50/90 değeri 0.5/1 μg/ml ve MİK aralığı 0.125-2 µg/ml iken; MRKNS'de MİK 50 / 90 değeri 0.25/0.5 μg/ml ve MİK aralığı 0.125-2 µg/ml olarak bulunmuştur. Sonuç: Ülkemizde henüz kullanımına başlanmamış 5. kuşak sefalosporin olan seftarolin MRSA ve MRKNS izolatlarında vankomisin ve linezolide yakın bir in vitro etkinlik göstermiştir. Bu bulgular, bu etkenlerin neden olduğu enfeksiyonların tedavisinde seftarolinin iyi bir seçenek olduğunu düşündürmektedir.

Background and aim Staphylococcus aureus (S. aureus) is one of the most common pathogens causing nosocomial and community-acquired infections with high morbidity and mortality rates. Fusidic acid has been increasingly used for the treatment of infections due to methicillin-susceptible S. aureus (MSSA) and methicillin-resistant S. aureus (MRSA). The present study aimed to determine the precise prevalence of fusidic acid resistant MRSA (FRMRSA), fusidic acid resistant MSSA (FRMSSA), and total fusidic acid resistant S. aureus (FRSA) on a global scale. Methods Several international databases including Medline, Embase, and the Web of Sciences were searched (2000–2020) to discern studies addressing the prevalence of FRSA, FRMRSA, and FRMSSA. STATA (version14) software was used to interpret the data. Results Of the 1446 records identified from the databases, 215 studies fulfilled the eligibility criteria for the detection of FRSA (208 studies), FRMRSA (143 studies), and FRMSSA (71 studies). The analyses manifested that the global prevalence of FRSA, FRMRSA, and FRMSSA was 0.5%, 2.6% and 6.7%, respectively. Conclusion This meta-analysis describes an increasing incidence of FRSA, FRMSSA, and FRMRSA. These results indicate the need for prudent prescription of fusidic acid to stop or diminish the incidence of fusidic acid resistance as well as the development of strategies for monitoring the efficacy of fusidic acid use.
