In vitro maturation (IVM) of oocytes recovered from ovariectomy specimens in the laboratory: a promising “ex vivo” method of oocyte cryopreservation resulting in the first report of an ongoing pregnancy in Europe

Abstract: IVM of oocytes obtained ex vivo during the processing of ovarian cortex prior to cryopreservation is a procedure with emerging promise for patients at risk for fertility loss, as illustrated by the reported pregnancy. However, more data are needed in order to estimate the overall success rate and safety of this novel approach.

“…In one patient all three oocytes collected were degenerated at collection, while in another patient three of five oocytes matured during IVM. A low MII rate was reported in prepubertal girls in previous study [7,10], but the collective experience with IVM of oocytes from prepubertal girls does not allow any conclusions to be drawn and should be expanded on in future studies.…”

“…A number of studies have shown that immature oocytes can be aspirated from small antral follicles visible on the surface of the ovary after excision usually with diameters exceeding 3 millimeter [4,5]. Two pregnancies resulting from cryopreserved embryos obtained from in vitro maturation (IVM) oocytes have recently been reported from women who recovered from cancer [6,7].…”

“…Mature eggs derived from IVM can be cryopreserved or fertilized and then cryopreserved as embryos. In vitro maturation (IVM) is another promising fertility preservation option, and importantly, pregnancies and live births have been obtained using this method (Chian, et al 2013, Guzman, et al 2012, Segers, et al 2015, Uzelac, et al 2015). …”

“…There are currently a variety of methods of fertility preservation, including cryopreservation of ovarian tissue (Segers et al, 2015), mature oocytes (Walls et al, 2015), and embryos. For unmarried women with breast cancer or malignant blood disorders, it is not rare to choose the resection and cryopreservation of ovarian tissue.…”
