“…This approach is often recommended despite the data that shows that fertilization rate and treatment outcome should be comparable with IVF and ICSI when the semen parameters are in the normal range and ICSI improves fertilization rates only when the semen parameters are borderline or when the morphology is poor [19][20][21][22]. Several studies have reported lower fertilization rates with IVF versus ICSI and a higher risk for complete fertilization failure with IVF among couples with unexplained infertility undergoing IVF following failed IUIs [15][16][17][18]23]. Ruiz et al, on the other hand found no significant difference between IVF and ICSI fertilization rates among women with unexplained infertility in their prospective randomized trial [24].…”

“…We have been using ICSI fertilization on half of the eggs and conventional IVF fertilization on the other half in cases of unexplained infertility. Other groups have recommended this approach as well [15][16][17][18].…”

“…ICSI is powerful for treating male-factor infertility that could not be assisted using conventional IVF. However, unexpected fertilization failures from IVF in couples with normal semen may occasionally occur (14,15). There are no absolute parameters or sperm function tests to predict fertilization outcome for apparently normal semen.…”

“…Some investigators attempted IVF-ICSI split on fresh sibling oocytes as the insemination method for normal semen to prevent fertilization failures (14,15). For couples with tubal or unexplained infertility, they found that the use of ICSI rescued 9-16% of cycles where IVF failed.…”

“…Plachot et al (2002) compared the fertilisation outcome of 58 first IVF couples with mild male infertility, among which 19 (32.8%) resulted in fertilisation after ICSI only and 39 (67.2%) with fertilisation in both IVF and ICSI. In another study performed by Hershlag et al (2002), complete fertilisation failure after conventional IVF occurred in 10 of 60 couples (16.7%) with unexplained infertility and in 2 of 50 couples (4%) with borderline semen characteristics. Results from the above reports suggest that fertilisation could be successfully achieved in the large majority of patients with mild male infertility or borderline semen.…”
