PURPOSE This in vitro study aimed to compare the accuracy of the conventional facebow system and the newly developed POP (PNUD (Pusan National University Dental School) Occlusal Plane) bow system for occlusal plane transfer in asymmetric ear position. MATERIALS AND METHODS Two dentists participated in this study, one was categorized as Experimenter 1 and the other as Experimenter 2 based on their clinical experience with the facebow (1F, 2F) and POP bow (1P, 2P) systems. The vertical height difference between the two ears of the phantom model was set to 3 mm. Experimenter 1 and Experimenter 2 performed the facebow and POP bow systems on the phantom model 10 times each, and the transfer accuracy was analyzed. The accuracy was evaluated by measuring the angle between the reference virtual plane (RVP) of the phantom model and the experimental virtual plane (EVP) of the upper mounting plate through digital superimposition. All data were statistically analyzed using a paired t -test ( P < .05). RESULTS Regardless of clinical experience, the POP bow system (0.53° ± 0.30 (1P) and 0.19° ± 0.18 (2P) for Experimenter 1 and 2, respectively) was significantly more accurate than the facebow system (1.88° ± 0.50 (1F) and 1.34° ± 0.25 (2F), respectively) in the frontal view ( P < .05). In the sagittal view, no significant differences were found between the POP bow system (0.92° ± 0.50 (1P) and 0.73° ± 0.42 (2P) for Experimenter 1 and 2, respectively) and the facebow system (0.82° ± 0.49 (1F) and 0.60° ± 0.39 (2F), respectively), regardless of clinical experience ( P > .05). CONCLUSION In cases of asymmetric ear position, the POP bow system may transfer occlusal plane information more accurately than the facebow system in the frontal view, regardless of clinical experience.

Protetik planlama ve tedavilerde estetik, fonasyon ve fonksiyonun yeniden düzenlenmesi hedeflenmektedir. Tedavi öncesi hasta talepleri ve tedavi sonrası şikayetler bunların öncelik sıralamasını etkileyebilmektedir. Birinci olgu sunumunda; 55 yaşında kadın tam dişsizlik olgusunun, alt üst (üst 4, alt 3 adet) implant destekli overdenture protezler ile olan tedavisi anlatıldı. Konvansiyonel adımlardan farklı bir protokolle tedavisi tamamlanan hastanın tedavisi pantografik kayıt cihazı ile eklemine ait dinamik verilerin açısal değerleri ile programlı tam ayarlanabilir artikülatör kullanılarak tamamlandı. İkinci olgu sunumunda; Mevcut protezlerinin yenilenmesini isteyen 65 yaşında kadın hastanın, tam dişsiz üst çenenin total protez ile, alt çenenin implant destekli overdenture protez ile olan tedavisi anlatıldı. Tedavi ilk olgu sunumuna benzer bir protokolle tamamlandı. Her iki vakanın final protezlerinde elde edilen pembe ve beyaz estetik memnuniyet vericidir. Ancak protezlerin final estetiğine rağmen artikülasyondaki erken temaslar memnuniyetsizliğe yol açmıştır. Her iki olguda da protezlerin teslimi sonrası hastaların protezlerinin belirli diş temaslarında yerinden oynadığı yönünde şikayetleri oldu. Gerekli dinamik stabilitenin sağlanabilmesi için her iki olgu sunumunda da direkt ağız içi tespitle dinamik faset onarımları yapıldı. 1. olguda dinamik stabilite 6 haftanın sonunda, 2. olguda ise teslim anında sağlanabildi.

Background Accurate integration of the dentitions with the face is essential in dental clinical practice. Here we introduce a noninvasive and efficient protocol to integrate the digitized maxillary dentition with the three-dimensional (3D) facial photo using a prefabricated modified computer-aided design/computer-aided manufacture (CAD/CAM) facebow. Methods To integrate the maxillary dentition with the 3D facial photo, the CAD/CAM facebow protocol was applied to 20 patients by taking a series of 3D facial photos in the clinic and integrating them in the laboratory. The integration accuracy of this protocol was compared with that of a valid 3D computed tomography (CT)-aided protocol concerning translational deviations of the landmarks representing maxillary incisors and maxillary first molars as well as the rotational deviation of the maxillary dentition. The intra- and inter-observer reproducibility was assessed, and the time of clinical operation and laboratory integration was recorded. Results This facebow-aided protocol generated 3D fused images with colored faces and high-resolution dentitions, and showed high reproducibility. Compared with the well-established CT-aided protocol, the translational deviations ranged from 0 to 1.196 mm, with mean values ranging from 0.134 to 0.444 mm, and a relatively high integration error was found in the vertical dimension (Z) with a mean ± standard deviation (SD) of 0.379 ± 0.282 mm. Meanwhile, the rotational deviations ranged from 0.020 to 0.930°, with mean values less than 1°, and the most evident deviation was seen in pitch rotation with a mean ± SD of 0.445 ± 0.262°. The workflow took 4.34 ± 0.19 min (mins) for clinical operation and 11.23 ± 0.29 min for laboratory integration. Conclusion The present radiation-free protocol with the modified CAD/CAM facebow provided accurate and reproducible transfer of the digitized maxillary dentition to the 3D facial photo with high efficiency.
