In vitro comparison of stiffness of plate fixation of radii from large- and small-breed dogs

Abstract: Plated radii of small-breed dogs had normalized stiffnesses equal to or less than plated radii of large-breed dogs. The complications typically associated with plating of radial fractures in small-breed dogs cannot be ascribed to an overly stiff bone-plate construct.

“…In fact, small breed radii plated with dynamic compression plates were less stiff than any plate used on large breed radii and cut-to-length plates on small breed radii. The authors of this biomechanical study concluded that complications typically associated with small breed radial fractures cannot be ascribed to an overly stiff radius-bone plate construct (12). These results suggest that other biological factors may contribute to the high incidence of osteopenia and delayed union in fractures of small dogs compared to their larger counterparts.…”

“…Highly stiff plates screwed to canine femurs may induce greater progression of osteoporosis compared to plates with greater elasticity (and greater bone loading); leading to the theory that stiff bone plating of radial fractures contributes to the high incidence of delayed union with these fractures (11). However, a recent ex vivo investigation into the stiffness of bone-plated radial fractures in cadavers failed to identify increased stiffness in small breed cadaveric radii compared to larger-breed radii following bone-plating and biomechanical testing (12). In fact, small breed radii plated with dynamic compression plates were less stiff than any plate used on large breed radii and cut-to-length plates on small breed radii.…”

“…6 A study observed no difference in the standardized stiffness of the radius after plate fixation between small and large breeds of dogs and that the potential for bone resorption in the radius of small breeds was less due to stress shielding. 7 However, it has also been reported that implants impair the blood supply to the cortical bone beneath the plate. 8,9 Cortical bone porosity was reported to have occurred within 12 weeks of internal fixation due to impaired blood flow, and that areas with impaired blood flow were associated with remodelling areas.…”
