Coagulase-negative staphylococci (CNS) cause the majority of post-cataract endophthalmitis, which can lead to anatomical and/or functional loss of the eye. This study reports the antibiotic susceptibilities of CNS isolates associated with acute post-cataract endophthalmitis cases and correlates antibiotic resistance with severity and outcome of infection in these patients. Clinical data (initial ocular examination, final prognosis, antibiotic treatment) and the antibiotic susceptibilities of the isolated CNS strains were obtained from 68 patients with post-surgical endophthalmitis recruited during a 7-year period by the FRench Institutional ENDophthalmitis Study (FRIENDS) group. The CNS strains displayed 100% susceptibility to vancomycin, 70% to fluoroquinolones, 83% to fosfomycin, 46% to imipenem and 18% to piperacillin. The most effective antibiotic combinations were fosfomycin plus a fluoroquinolone and imipenem plus a fluoroquinolone, which were considered adequate in 80% and 58% of patients, respectively. Methicillin resistance was significantly associated with older age (p 0.001), diabetes mellitus (p 0.004), absence of fundus visibility (p 0.06), and poor visual prognosis (p 0.03). Resistance to fluoroquinolones was significantly associated with absence of fundus visibility (p 0.05) and diabetes mellitus (p 0.02). This large prospective study demonstrates that methicillin resistance and, to a lesser extent, fluoroquinolone resistance in CNS strains causing postoperative endophthalmitis are both prevalent in France and associated with a poorer visual prognosis. These results emphasize the need for an effective surveillance of this antibiotic resistance and the development of new diagnostic tools for rapid detection for early optimization of antibiotic therapy in endophthalmitis patients.

Purpose: To characterize the conjunctival flora of patients receiving antibiotics before intravitreal (IVT) antivascular endothelial growth factor injections. Methods: We studied 31 eyes of 30 patients with neovascular age-related macular degeneration scheduled to receive IVT antivascular endothelial growth factor injections. The patients were all given a preinjection of topical ofloxacin 3 days before the injections. Conjunctival cultures from the injection eye were collected before antibiotic prophylaxis (sample A) and on the injection day, both before (sample B) and after (sample C) povidone-iodine (PVI) application. Results: Culture positivity was significantly different between samples A (50%) and C (4.5%; p < 0.001) as well as samples B (30.4%) and C (4.5%; p = 0.015), but not between samples A and B. Positive cultures were mainly coagulase-negative staphylococci (CNS; 57% of sample A, 62.5% of sample B and 100% of sample C) susceptible to vancomycin (100%), moxifloxacin (90%), levofloxacin (70%) and ofloxacin (60%). Conclusion: In our study, most of the bacteria isolated were CNS. PVI application but not antibiotics reduced culture positivity.

SummaryPurpose: To investigate the pathogens associated with bacterial conjunctivitis and the in vitro antibiotic sensitivities of these bacteria. Material and Method: Forty-seven (27 female, 20 male) patients with a mean age of 43.7±26.4 (range: 1-84) years, who referred to our out-patient clinic with complaints of burning sensation, stringy discharge and hyperemia between December 2008 and March 2010, who were diagnosed with acute bacterial conjunctivitis, and had no history of any systemic or topical antibiotic use were included in the study. Samples were taken from both eyes using cotton swabs, cultured onto chocolate and blood agar, and prepared for Gram staining. The identification of organisms was performed by Vitek2 compact system (bioMerieux, France). Antibiograms were evaluated according to the Clinical and Laboratory Standards Institute (CLSI) criteria by disc diffusion method. Results: Twenty-nine of 47 (61.7%) samples were culture-positive. The most common isolated bacteria were coagulasenegative staphylococci (16 cases, 55%). For them, the most sensitive antibiotics, given in decreasing order, were as follows: vancomycin (100%, 21/21), netilmicin (95.7%, 22/23), chloramphenicol (92.6%, 25/27) and tobramycin (91.3%, 21/23). Fluoroquinolones were relatively less sensitive: ofloxacin (75%, 21/28), moxifloxacin (75%, 18/24), ciprofloxacin (73.1%, 19/26). Discussion: Netilmicin, chloramphenicol and tobramycin were found to be more sensitive compared to fluoroquinolones in the treatment of bacterial conjunctivitis. Using these antibiotics as an empirical treatment, taking conjunctival culture of particular cases prior to treatment, and antibiotic switching according to antibiogram will be the most reasonable approach in case of no response to treatment.
