In vitro activities of cefotaxime, ceftriaxone, ceftazidime, cefpirome, and penicillin against Streptococcus pneumoniae isolates

Abstract: The in vitro activities of four extended-spectrum cephalosporins and benzyl penicillin were evaluated against 698 clinical isolates of Streptococcus pneumoniae, including 130 (19%) penicillin-intermediate and 84 (12%) penicillin-resistant strains. Cefotaxime and ceftriaxone were essentially identical in their antipneumococcal activities: both were active against penicillin-susceptible strains and most penicillin-intermediate strains.Cefpirome was twice as potent as cefotaxime and ceftriaxone against penicillin… Show more

“…Alternatively, new therapeutic compounds must be evaluated. In our study, the MIC 90 s of meropenem, imipenem, ceftriaxone, and vancomycin of penicillin-sensitive, -intermediate, and -resistant pneumococci were consistent with those reported elsewhere (3,27). On the basis of the MICs, imipenem was the most active ␤-lactam agent.…”

“…The clinical efficacy of a single intramuscular injection of 50 mg of CRO per kg of body weight in pediatric patients with uncomplicated otitis media is equivalent to those of standard oral 7-or 10-day amoxicillin (22,36), 10-day amoxicillin-clavulanate (37), and 10-day trimethoprimsulfamethoxazole (4) therapy. The recent emergence of clinical cephalosporin-resistant S. pneumoniae strains, revealed by treatment failures in patients with pneumococcal meningitis (10-12, 17, 24, 31), although initially considered an epiphenomenon (15), now appears to be a continuum in penicillinbinding protein alterations in penicillin-resistant strains (5). This extension of resistance must be taken into account in the assessment of CRO therapy.…”

“…Additionally, the reported ceftazidime resistance is very interesting given the pan-susceptibility to all simple b-lactams, which requires further investigation. Similar instances of streptococcal strains that are penicillin-susceptible and resistant to ceftazidime have already been reported (Barry et al, 1995), and ceftazidime resistance has been attributed to mutations in several bla CTX-M determinants (Welsh et al, 2005).…”
