Improving the efficiency of cognitive-behavioural therapy by using formal client feedback

Abstract: Frequently asking feedback from clients using the ORS/SRS does not necessarily result in a better treatment outcome in CBT. However, for an equal treatment outcome significantly fewer sessions are needed within the Feedback condition, thus improving efficiency of CBT.

“…A Cochrane report, which included 17 studies involving 8,787 adult participants treated for mental health disorders, found that ROM and TAU conditions did not significantly differ (Kendrick et al, 2016). More specifically, Janse deJong, Van Dijk, Hutschemaekers, and Verbraak (2017) found that ROM was not more beneficial for 1,006 outpatients than TAU in general with the exception of those suffering from mood disorders who benefited more from ROM. In a Danish study of 159 clients suffering from eating disorder, there was no significant difference in therapy outcomes between ROM and TAU conditions (Davidsen et al, 2017).…”

“…Inconsistency can be caused by plausible heterogeneity on how therapists actually use ROM systems, by differences on the type of measures included in the system, or by the algorithms and formats of feedback it provides, the type of therapy, among many other factors. Research is needed that unveils how feedback is used and the mechanisms by which feedback influences outcomes (Amble et al., ; Janse et al., ; Moltu et al., ). Greenhalgh, Long, and Flynn () hypothesized that the lack of consistent impact of outcome feedback into practice may be due to the nomothetic nature of the assessment tools used, which provide too general information, whereas therapeutic processes are individualized.…”

“…Noteworthy was the finding that giving ROM feedback to both therapist and his/her supervisor (including joint discussion about it) seemed not more effective than giving ROM feedback to therapists alone [6]. Mixed results were found regarding the cost-effectiveness of ROM and the positive impact of ROM on satisfaction of patients with provided care [10, 42,60].…”

“…Research also suggests that providing feedback from multiple sources (both wellbeing and affective psychological distress) enhances patient outcomes more, compared to single source feedback ([only wellbeing; [43]). Furthermore, ROM may improve the patient-therapist communication and the process quality of treatment (i.e., presence of treatment plan) and ROM feedback to therapists can lead to fewer treatment sessions [42]. Noteworthy was the finding that giving ROM feedback to both therapist and his/her supervisor (including joint discussion about it) seemed not more effective than giving ROM feedback to therapists alone [6].…”
