BACKGROUND: Care coordination is designed to provide holistic care across multiple health care delivery systems in a timely manner. Care coordination involves a multi-disciplinary team and a collaborative approach when walking alongside the consumer. Care coordination offers a health care delivery model that is entrenched in several key delivery principles that are based on understanding a consumers' experience and formulating a shared-care pathway in a respectful manner. OBJECTIVE: The aim of this study was to explore consumer experiences of care coordination within Barwon Health's Hospital Admission Risk Program (HARP) located in Geelong, Victoria, using a qualitative descriptive approach. METHOD: The study adopted an inclusive and coproduced research design. Semi-structured interviews were conducted with a purposive convenience sample of six consumers living with chronic health conditions and other complex psychosocial needs. Participants were asked about their experiences related to accessing the service, communication, health and supports before and after accessing the service, and 'their' lived experiences. RESULTS: Five meta-themes were identified: (1) experiencing authentic, values-based care, (2) collaborative care and working together, (3) gaining independence, (4) improved health and quality of life, and (5) limited understanding of HARP at the start. Overall, participants' experiences were positive, assistance in improving health literacy, quality of life, and sustainable supports. While gains were experienced, most of the participants identified that their knowledge of HARP was limited when services were introduced and 'their' enrolled voluntary participation. CONCLUSION: This research addresses the knowledge gap related to consumer experiences of care coordination. Findings offer an insight in providing personcentred, authentic and values-based care. The findings highlight the need to improve consumer and referrer knowledge and provides an understanding of care coordination service delivery principles that are: inclusive, individualised, and holistic and embodies a creative ideology to break down roadblocks in a consumer's health care journey.
