Improving Outpatient Follow-Up Through Innovative Appointment Scheduling at Emergency Department Discharge

Abstract: This quality improvement (QI) project evaluates a comprehensive, patient-centered plan to reduce the emergency department (ED) revisit rate by improving outpatient follow-up after initial ED visits.
