Improving interobserver variation in reporting gadolinium-enhanced MRI lesions in multiple sclerosis

Abstract: Gadolinium-enhanced MRI is a sensitive and objective means to monitor disease activity in multiple sclerosis (MS). We evaluated the interobserver agreement and the value of observer training in reporting enhancing lesions from serial MRI. Scans of 16 MS patients were evaluated by five inexperienced and five experienced observers before and after consensus formation and training. The number of lesions at baseline, and the number of new and persistent lesions at follow-up were scored. For each condition, weighte… Show more

“…A reduction of these variabilities was shown after more precise guidelines were given for the different types of lesions (Barkhof et al, 1997;Filippi and Rocca, 2007). Similar efforts would also be interesting for the segmentation of lesions.…”

“…MRI assessment used triple-dose gadolinium for the enhancement of lesions on brain scans. 53,54 On the basis of EDSS, clinical improvement was defined as at least a 0.5-point score reduction compared with baseline. Progression was defined as a 0.5-point score increase compared with baseline.…”

“…27 The MRI procedure was standardized in all participating centers and was performed according to established criteria. 28 Images were sent to the Neuroimaging Research Unit, Scientific Institute and University, S. Raffaele Hospital, Milan, Italy, where they were examined and quantified by an experienced observer masked to treatment phase. The number of Gd-enhancing lesions were counted.…”
