Improving Cancer Patient Care with Combined Medication Error Reviews and Morbidity and Mortality Conferences

Abstract: Background: To reduce the occurrence of medication errors, a systemic approach was developed combining anti-neoplastic medication error reviews and morbidity and mortality conferences (M&MCs).
