Background：Because Tamoxifen Retinopathy and Macular Telangiectasia Type 2 (Mac Tel-2) have similar manifestations, many researchers are identifying and exploring the mechanisms of the two diseases. Here we report a patient with selective estrogen receptor modulators(SERMs)-induced retinochoroidopathy. And she has a history of tamoxifen and toremifene use , presenting the classic phenotype of TR. It is the first time, we use comprehensive examination to observe one patient. Case presentation: A 52-year-old woman presented gradual and progressive visual acuity decrease of both eyes about 1 year. She had received oral tamoxifen followed by toremifene for 57 months. Indocyanine green angiography(ICGA), optical coherence tomography angiography(OCTA), electrooculogram(EOG), revealed salient distinct from Mac Tel-2 phenotype in patient’s choroidal. The patient’s ocular vessels did not show any tendency to proliferate, so we serve the cessation of anti-estrogen drugs as her treatment. Conclusion: Combined with the positive results of increasing choroidal thickening, ICGA hypofluorescence, and decreased Arden ratio, SERMs-induced retinopathy is considered to be a type of retinochoroidopathy.

The purpose of this case report is to present the first case of neratinib maculopathy. We describe the initial presentation, baseline characteristics, imaging findings, and outcomes. The case report is accompanied by a thorough literature review including possible mechanisms of tyrosine kinase inhibitor (TKI) maculopathy.Neratinib is a novel TKI that is commonly used in the treatment of breast-associated malignancies. Neratinib toxicity presents similarly to macular telangiectasia type II but differs with the fine granular hypofluorescent areas spanning the limit of the posterior pole and vascular arcades as well as the nasal aspect of the optic nerve.We report a case of suspected macular toxicity secondary to neratinib. Concomitant use of neratinib in conjunction with docetaxel and other chemotherapeutics with known retinal side effects should alert clinicians of an increase in the risk of macular toxicity. Albeit commonly reported ocular side effects of TKIs, maculopathy is a rare and potentially overlooked side effect. Patients that have planned chemotherapy should undergo a baseline retinal examination.

The aim of this review was to identify a new potential explanation for the development of macular holes in relation to the female sex and to explain the possible underlying pathways. This approach was based on the evaluation of anatomical, physiological, and morphological analyses currently available in the literature. The findings showed that estrogen exerts a protective effect on the neuroretina and may influence Müller and cone cells. Both cell types are responsible for the building of the fovea structure. However, this protection may be lost due to the sudden decrease in estrogen levels during menopause. In conclusion, the fovea cones, through its sensitivity to estrogen and high energy consumption, may be very vulnerable to damage caused by a sudden changes in the concentration of estrogen in menopausal females. Such changes may result in cone degeneration, and thus a destroyed structure of the fovea, and may lead to the development of a hole in the fovea, as in the case of macular holes. This review revealed that under the decreasing influence of estrogen may cones play a key role with regard to the etiology of the development of macular holes. This aspect may be of strategic importance in prophylactic therapy for the prevention of the development of macular holes in premenopausal females or after ocular trauma.
